SPECIAL NOTICE TO SUBORDINATE
CHAPTER SECRETARIES
Below you will find some practical suggestions for making out your Annual Report. Please follow them carefully.
1.Type your report if at all possible. This may be facilitated by removing the fastener,and by using a carbon, both copies can be made at the same time. Then fasten the report together again. If you cannot type, print or write the names very carefully, using particular care with the m's, n's, u's, o's and a's, as these are the most confusing in handwriting.
2. List all names alphabetically in your membership list and also on your lists of initiates, affiliates, suspensions, demits, and deaths. This aids greatly in checking the report.
3. Please indicate by the use of a star, whether a member is a Past Matron, or a Past Patron. If a member is a PM or PP of another chapter and now affiliated with your chapter, use a star and the number of her/his former chapter. Please indicate Miss or Mrs.
4. Be sure that your recapitulation agrees with the various items from which it is made up; for example, you should have the same number of names listed under the various headings of initiates, affiliates, demits, suspensions, and deaths as you show in your recapitulation for these sections. Your membership list in the front of the report MUST contain the same number of names as shown under "total present membership" in your recap. Please check this carefully.
5. Check your membership list against the one shown in the last membership roster for spelling, initials or omissions.
6. Corrections: if you have corrections in spelling, initials, omissions or other discrepancies, space is provided for making the notations. The "change in name" section is to be used only for those who have changed name by reason of marriage. It is not to be used for corrections.
7. The check for your per capita tax mustaccompany your report. Per capita tax isfigured on every member except those who are exempt, according to Sect. 1, pg 25 of the Code.
8. Return one copy of the complete report to the Grand Secretary on or before May 30th. Be sure to put on enough postage.
9. The WM or AM must examine and sign thereport. Secretary should then affix theChapter Seal on the last page before mailing to Grand Secretary.
