BRIEF RESUME OF THE INTENDED WORK

6.1 NEED FOR THE STUDY

Kaphaja Pratishayaya is an irritating disease commonly observed in general practice having 30 - 40% prevalence all over the world. Kaphaja Pratishayaya is caused by the vitiation of Kapha Doshaand has the symptoms such as Shukla Sheeta Nasa Srava, Kandu, Shoona-Akshi-Koota-Shotha andShirogaurava2. These symptoms of Kaphaja Pratishayaya can be correlated to chronic simple rhinitis.Chronic simple rhinitis is inflammation of the inner lining mucosa characterized by, nasal obstruction, nasal discharge which trickles in the throat, heaviness in the head3.

Pratishayaya, if left untreated causes dreadful complications like Rajayakshma2, hence there is a need to find an efficient treatment in the management of Kaphaja Pratishayaya. In Ayurvedic classics many treatment principles are advised for Pratishaya includingNasya karma which has been given utmost importance in its management.

Pipalyadi Avapeedana Nasya has been indicated for Kaphaja Pratishyaya3. It is having the ingredients as Pippali, Shigrubeeja, Maricha, and Vidanga which act as Kapha Vata Shamaka. Pippali is Rasayana and included in Shirovirechana Gana and does Kapha Nisarana.Shigru beeja also doesShirovirechana and has Kaphaghna property. Maricha has Nadibalya and Kaphanisraka property.Vidanga isKrimighna and Shirorogahara. Taking into consideration all thesebeneficial actions of Pipalyadi Nasya this drug is selected 4, 5.

To evaluate its efficacy in the management of Kaphaja Pratishyaya. This Nasya will be given for 3 days in one group of patients and for 7 days in other group of patients to ascertain the exact duration of treatment for providing cure to the patients.

6.2 REVIEW OF LITERATURE

Pratishayaya is the pradhana roga explained in the context of Nasa Roga6.

Kaphaja pratishayaya is one among the five types of pratishayaya6a.

The specific Nidanas like exposure to Avashyaya, Anila ,Rajo, Dhooma, Dharana of Vegas like Chardi, Ashru, Jrumbha and Atyambupana leads to Kaphaja Pratishayaya7,8,9.

The Samprapti is explained by our Acharyas that the Vitiated Vatadi dosas gets lodged in the Siras and then propogates to Nasa and produces irritation over the Nasa Shleshma Kala which leads to Pratishayaya10.

The lakshanas of kaphaja pratishayaya are Shukla Sheeta Nasa Srava, Kandu, Shoona-Akshi Koota-Shotha and Shirogaurava, 11, 12, 13.

Nasya is explained as the treatment principle of pratishayaya in Charaka Samhita14.

Pippalyadi Avapeedana Nasya has been mentioned in Bhaishajya Ratnavali, Chakradatta, Vangasena15, 16, 17.

Pippali, Maricha, Shigrubeeja and Vidangaare recommended specially for Kaphaja pratishayaya. The above drugs are having kaphahara properties and hence may prove to be a useful remedy for management of kaphaja pratishayaya.

No research work has been carried out in kaphaja partishayaya till date.

OBJECTIVES OF THE STUDY.

1. To evaluate the effect of Pippalyadi Avapeedana Nasyagiven for 3 days in the Management of Kaphaja Pratishayaya.

2. To evaluate the effect of Pippalyadi Avapeedana Nasya given for 7 days in the Management of Kaphaja Pratishayaya.

3. To ascertain that whether 3 day’sduration of Nasya provides same relief as that of 7 days

7.0 MATERIALS AND METHODS

7.1. Source of data

a) Patients

Patients with classical features of kaphaja pratishayaya will be selected from OPD of ShalakyaTantra of SDM College of Ayurveda and Hospital Hassan.

b) Trail Drug

The pippalyadi Avapeedana Nasya will be prepared as per the following methods as mentioned in classics.

1. Pippali.

2. Shigrubeeja.

3. Vidanga.

4. Maricha.

The above mentioned dry drugs will be taken in equal quantity and soaked indouble the quantity of the water and will be kept for one day and night.Then Kalka will be prepared and will be squeezed through a piece of cloth and the Svarasa thus obtained will be filled in adropper and will be administered as Nasya. Fresh juice will be prepared on each day of Nasya.

7.2 Methods of the collections of data:

a) A clinical study will be conducted on thirty patients of Kaphaja Pratisyaya.

Diagnostic Criteria

Diagnosis will be established on the basis of history, signs, symptoms and Anterior Rhinoscopy.

b) Inclusion criteria:

1)Patients between the age of 7 years and 70 years will be selected irrespective of sex, occupation, religion and socio economic status.

2)Patients suffering from Kaphaja pratishayaya will only be taken up for the study.

3) Patients fit for Nasya.

4)Chronicity of disease less than one year.

c) Exclusion criteria:

1) Age less then 7 years and above 70 years.

2)Other than kaphaja pratishayaya (vata. Pitta, Rakta, Sannipata).

3)Patients suffering from Chronic simple Rhinitis associated with

Tonsilitis, Pharyingitis etc.

4)Chronicity of the disease more than one year.

Treatment Group

30 patients of kaphaja Pratishyaya will randomly and equally be divided into the following two groups

The patient will be administered nasya early in the morning with empty stomach.

Purva Karma:SthanikaMukha Abhyanga with plain Tila Taila and Sthanika Sveda overMukha Pradesha with towel soaked in Sukoshna Jalawill be done as Poorva Karma.After Nasya with pippalyadiswarasa 6 drops and in each nostril.Paschat Karma asKavala with Sukoshna Jalawill be done.

1) Three Days Nasya :Group: 15 patients of this group will be administered 6 drops of Pippalyadi Avapeedana Nasya in each nostril for 3 days. Nasya will be given in the morning, once a day on empty stomach.

2) Seven Days Nasya :Group: The patients of this group will be administered 6 drops of Pippalyadi Avapeedana Nasya for 7 days, once in the morning on empty stomach.

Pathya and apathya of Nasya karma will be explained to the patient during the course of treatment.

d) Follow up:

The patients will be followed up at an interval of fifteen days for a period of two months.

e) ASSESSMENT CRITERIA:

Patients will be assessed with subjective and objective parameters formulated for kaphaja Pratisyayabefore and after treatment.

3)Heaviness of head

4)Headache

5)Itching sensation in the throat, palate and lips

Objective Parameter:

Anterior Rhinoscopic Findings.

1)Nature of secretions.

2)Color of nasal mucosa.

3)Size of turbinate.
