Instructions:

  1. At the completion of each phase of the orientation, you and your supervisor are required to sign each of the relevant areas.
  2. This will indicate that you have been trained in accordance with the project HSE Management System.
  3. On completion of the orientation questionnaire, hand the form to your supervisor, who will return it to the HSE Representative.
  4. If at any time you are unsure of anything you are being told or shown, do not hesitate to ask.
