Program Description and Reporting Requirements

PROGRAM NAME:Environmental Health Risk Assessment/Communications

PROGRAM CODE:040

FUNDING SOURCE: Preventive Healthand Health Services Block Grant Funds

PURPOSE:To identify people at risk for health problems from exposure to hazardous substances in the environment, determine relationships between exposure to hazardous substances and human diseases, and reduce or eliminate exposures of health concern. Funding is distributed through the Floyd, Bibb, and Chatham County Boards of Healthfor the benefit of counties in the regions indicated on the attached map.

FUNDING REQUIREMENTS:

Restrictions:

Funds are used to build regional capability at local health departments in dealing with communities at risk for exposure to toxic or hazardous materials, to provide a source of environmental health information to communities and address other general environmental health concerns.

Allowable costs and expenses are salary, fringe, and travel for three regional (headquartered in Rome, Macon, and Brunswick district offices) staff to educate the public, community leaders,and localofficials regarding exposure to hazardouschemicals in air, water, and soil in defined regions of the State. Persons supported by allowable salary and fringe may provide assistance in other environmental health programs as time permits.

Indirect costs may not be charged to this program unless a cost allocation plan has been approved by the Department’s Division of Finance.

Deliverables:

All program activities are conducted using formats, guidance, and technical assistance from, and evaluated by, the Centers for Disease Control, Agency for Toxic Substances and Disease Registry under Cooperative Agreement number 5U61TS000173-03.

Primary

Provide detailed documentation and reports concerning possible hazardous materials exposures in contractor’s region.

Conduct environmental health assessment of individuals or communities exposed to hazardous substances per protocol.

Attend community meetings and provide education and/or training related to environmental health assessments.

Attend all meetings, trainings, and conference calls with Georgia Chemical Hazards Program staff as directed.

Secondary

Provide assistance in other environmental health programs including but not limited toon-site sewage disposal, food service, tourist accommodations, lead, or drinking water.

Develop capacity through development of emergency preparedness and response activities.

PERFORMANCE MEASURES:

Objective: Ensure 100% of environmental health assessments and technical reports are conducted and written in approved format per CDC, ATSDR and EH Section protocols and provide three, or the equivalent of three, health consultations (1 public health assessment; 3 health consultations; 9 written technical assistance reports to individual/groups, i.e cancer cluster investigations and health study activities, 3 site-specific activities, or a combination thereof).

Objective:Under state direction ensure community involvement by providingup to 6 professional trainings for community groups and health educational outreach programs for citizens by attending up to 6 site-related community advisory group/public meetings.

ALLOCATION METHOD: Funds are distributed regionally (Floyd, Bibb, and Chatham County Boards of Health) to provide risk communication/assessment services coverage across the state. Regional locations are determined based on superfund sites associated with the ATSDR grant deliverables.

REPORTING REQUIREMENTS:

  1. A quarterly programmatic/performance statistical report is to be submitted electronically no later than the 15th day after the end of each three month period during the term of this agreement. The report is to be a county-generated narrative analysis of project accomplishments to include percentage of completion, problems, assistance needed and other elements to assist the EH Section in program evaluation.
  2. Included in the quarterly reports will be all performance measures, cost schedules, FTE allocation, itemized expenses and description of assistance activities provided to other environmental health programs.
  3. Submit by May 15th, an annual budget report detailing expenditures. Budget may be provided in UAS.
