1. Purpose. All Navy personnel nominated to mobilize and elect to take Active duty special work orders (ADSW)or temporary duty (TDY) for over 30 days must meet certain criteria per reference (a) through (c) and the references therein. All members who are elected to mobilize mustbegin their process at no later than the 180 day mark from their ready load date (RLD) with the NOSC Los Angeles Medical Department for medical clearance and to giveNOSC LA medical time for waiver delays.

2. Per reference (a) and (b), the following are proper procedures for mobilizations as soon as the members are notified:

  1. DEERS Enrollment
  1. Tricare enrollment is automatic, however, members should ensure this has been completed if they do not receive notice in the mail within 2 months of receipt of orders for themselves and family members if they elect to enroll their family (this is not an option for activated reservist).
  1. Health and Dental records are reviewed immediately by the medical department for currency and any issues that may adversely affect the mobilization.
  1. Preventive Health Assessment within 90 days of ready load date (RLD).
  1. Dental exam

(1)Class I OR IIexam within 90 days of RLD

(2)Panogram within 5 years RLD

(3)Bitewings within 1 year 6months of RLD

f. Labsand tests

(1)Fasting lipid with glucoseat the 180 day mark of

notification

(2)HIV (120 days of RLD)

(3)PSA (Males over 50)

(4)Liver panel for members on prophylactic treatment

(5)Pap smear (they must have 3 consecutive readings that are

negative prior to RLD, if member requires follow on care in

less than one year their orders will be canceled

immediately)

(6)Normal Mammogram for members over the age of 50 (within one

year of RLD)

(7)TSH drawn (members with thyroid disease only)

(8)Any other lab deemed necessary

(9)G6PD (ONE TIME LAB)

(11)Blood type/RH Factor (ONE TIME LAB)

(12)DNA-ensure it has been registered with the repository

(13)PPDwithin one year of RLD (Chest X-ray for PPD convertors

and proof of treatment is documented in their record)

(14)Audiogramona 2215 OR 2216 within one year of RLD

h. Currenteyeexamwithin 2 years of RLD

(1)Members who require glasses must obtain (2) pairs of

glasses, (1) GMI, (1) Ballistic

i. Allergy tags if required for allergies

j. Immunizations per each AOR.

  1. Within 30 daysof ready load date, ensurethefollowingarecompletedand signed per the 1300/3A:

(1)DD 2807 Reportof History

(2)DD 2795 Pre-deployment health assessment completedon the

EDHA website

(3)1300/4 withspecific AOR screening forms

(4)NAVPERS 1300/21 MedicalSuitabilityCertification

(5)If a member is in a physical risk class “B”, appropriate

waivers are required.

l. If member is on prescribed medication, ensure they have

acquired 180 days’ worth of medication and are aware of the

Tricare mail order pharmacy. Ensure waivers are approved and

placed in the deployment record.

m. NAVPERS 1300/4 Expeditionary Medical andDental Screening for

Individual Augmentees andSupport Assignments to overseas contingency Operations (OCO)

  1. If there is an adverse answer to any medical form or lab, review the issue further. If time permits seek treatment and/or a waiver for acceptable areas, see reference (b) and contact the NMPS immediately. When all avenues are exhausted and the members are not fit to deploy, cancel the orders and place them in a TNPQ status to try and resolve the issue. Any member who fails to meet mobilization readiness due to medical are required to have a Medical Retention Review package.
  1. Create a deployment health record by the 30 day mark of the RLD. Deployment record must include the following documents:
  1. Appropriate NAVMED 6150/10-19 for member complete per MANMED

Chapter 16

2. Last 3 Years of SF 600 entries

3. Updated IMR from MRRS

4. Any food/drug allergies documented with medical warning tags

5. NAVMED 6120/4

6. Periodic Health Assessment (PHA) completed within 90 days

of deployment

7. Updated DD Form 2766

8. Report of Medical History (DD Form 2807-1)

9. Audiogram (DD Form 2215 or DD Form 2216 completed within 12

months of deployment)

10. Immunization record

11. PPD or PPD Converter Annual Questionnaire/Proof of INH

completion

12. Blood type and Rh factor lab chit

13. Sickle trait lab chit

14. DNA sample collected and registry date recorded

15. G6PD results with date. If deficient, Red Dog Tags and the

statement "NO PRIMAQUINE

16. HIV results

17. Eye examination within 2 years of deployment.

18. If correction required, corrective prescription current

(within one year) and on DD Form 771.

19. 2795, Pre-Deployment Health Assessment within 60 days of

deployment

20 NAVMED 1300/4

21. Dental exam completed in the last 90 days

22. Printed copy panoramic radiograph within 5 years

23. Printed copy current bitewings within 1 year 6 months of RLD

24. Printed copy all periapical radiographs specific to previous

complex dental procedures such as endodontics,

prosthodontics, and periodontal treatment if applicable

25. PERS PRC Message and BUMED letter if applicable

26. Any associated waiver

27. AOR

28. Small Arms

29. Any PFA waivers within last 3 years

30 Any additional supplemental forms

31. NAVMED 1300/5, Pacific Command (PACOM) (if required).

32. NAVMED 1300/6, Korean Peninsula(if required).

33. NAVMED 1300/7, European Command (EUCOM)(if required).

34. NAVMED 1300/8, Africa Command (AFRICOM)(if required).

35. NAVMED 1300/9, Joint Task Force (JTF) Guantanamo Bay

(GTMO)(if required).

36. NAVMED 1300/10, West Pacific & Okinawa, (With Extended

Field Exposure)(if required)..

37. NAVMED 1300/11, United Nations Mission(if required).

38. FEMALE SPECIFIC REQUIREMENTS:

a. Pap smear results

b. Mammogram results

c. Documented negative pregnancy results within 30 days of

deployment (Mark N/A for documented hysterectomy.)

3. Per reference (c), members are to check in to the medical department when they have elected to take on ADSW or TDY orders for completion of the required check list. Anyone who is on limited duty or requires waivers must have each issue cleared before departing NOSC LA.
