RE: Medical Evaluation Reports at Eligibility Team Meetings - Procedures

Effective immediately and until further written notice, school nurses are responsible for bringing

to all eligibility team meetings the original and one hard copy of the medical evaluation completed by Suffolk Public Schools for the purposes of a student’s special education eligibility determination.

Scanners have previously been purchased for every school from the special education budget for the use by child study, eligibility and Individualized Education Program (IEP) team meetings (see attached listing of locations). Special education compliance specialists are responsible for ensuring that the medical evaluation document is scanned and attached to the student’s electronic record in IEP Plus, either during the actual eligibility team meeting or immediately after the conclusion of the day’s scheduled eligibility team meetings. The hard copy is to be provided to the parent(s) as part of the standard eligibility report packet and the original kept on file in the school’s clinic.

Any compliance specialist requiring initial or refresher training in the use of the scanner may meet with Julie Masters, Assistive Technology Specialist, at 3:30 p.m. on Wednesday, December 15, 2010 at Elephant’s Fork Elementary School in mobile unit #2. Otherwise, compliance specialists are to schedule an individual training appointment with Mrs. Masters.
