The application package should include:

  • Two complete printed applications (including any attachments). If your applicant appraisals and/or transcripts are being delivered separately to the FCCF office or if given to you in a sealed envelope, we will make an additional copy for you.

Applications must be received at the FCCF office by 4:00 PM on Thursday, March 1, 2018. Applications received after this date will not be considered. Please be certain all questions are answered completely or this application will not be considered.

APPLICANT APPRAISAL

STUDENT: Print two appraisals and distribute to two adults. One applicant appraisal may be completed by a school administrator or teacher, and the remaining one from a community or religious leader, employer, or another person in a position of authority who knows you and your accomplishments. Members of the FCCF Board of Directors, FCCF staff or members of your family are not permitted to complete appraisals. Please remember to collect the appraisals from the adults and include them with your application. If they choose to, the appraisers may mail their appraisals to the FCCF Office at 527 Main Street, Brookville, IN 47012. Appraisals must be received at the FCCF office by 4:00 PM on March 1, 2018.
