Fourth Quarter Celebration Day Interest Survey

Please do not send money at this time.

Our Fourth Quarter Celebration Day is scheduled for Thursday, June 1, 2017. This survey will determine interest, help guide the planning of the trips, and assure that there are enough spaces for students to attend their desired trip, therefore, once it is turned in choices are final. The form is due by Friday, February 17, 2017. If it is not received by then, students may be assigned to a trip.

The three options are listed below. Keep in mind, that although the trip to Great America is reserved for 8th grade, 8th grade students can choose a different option instead. 6th and 7th grade students can select from the Kalahari or the Helium House/Miller Park Tour options. Please note that in order for the Helium House/Miller Park Tour trip to occur, there must be A MINIMUM of thirty students. Because of that, you will need to have a second option listed if you choose the Helium House/Miller Park Tour.

In order to be eligible to attend, you need to have all passing grades [no Fs], your conduct and effort grades must be either an “O”, “S”, or an “N” [no Us], and your cumulative grade point average must be above a 1.7 as of May 25, 2017.
