Grant Application Guidelines

“Working Together To Improve Our Schools”

Please read carefully to fully understand the grant expectations and guidelines. Only grant requests that positively impact academic instruction and directly benefit student achievement will be considered. The number of grants awarded will depend upon available funds.

Application Eligibility:

1.  Foundation funds are limited to Fort Dodge School District employees.

2.  The purpose of the grant project must complement the District's mission and curriculum goals.

3.  The grant project must have a designated time frame for completion.

4.  The grant project must have measurable objectives.

5.  If the grant is for a Department or Building, a project director must be named to assume overall administrative responsibilities for the grant project and all related correspondence.

6.  Recipients must adhere to all District financial guidelines and policies.

Application Requirements:

To be considered for funding, the application must:

1.  Be completed in full.

2.  Be free from any identifying information (applicant or school name) other than the cover page.

3.  Be grammatically correct and free of spelling errors.

4.  Be typed and printed.

5.  Describe a method to evaluate the success of the project.

6.  Be in compliance and approved by the Principal/Supervisor and Superintendent for curriculum and instruction.

Application Deadline:

Applications must be submitted to the Superintendent by the 15th of November for Fall requests and by the 15th of April for Spring requests.

Notification of Recipients and Funds:

1.  Proposals shall be competitively reviewed by the Foundation.

2.  Recipients will be notified within 60 days from submission of the proposal.

3.  Funds for approved grants will be deposited into District account.

4.  Any unused funds will be returned to the Foundation after the completion of the grant project.

5.  Products or equipment purchased with the grant funds becomes the property of the FDCS District.

6.  Recipients must provide a final evaluation and financial report at the conclusion of the grant project.

7.  Recipients may be asked to attend a Foundation Board and/or School Board meeting to discuss their grant project.

8.  Recipients may be asked to allow for photographs of the project to be taken.

Process:

1.  Staff member:

c.  Complete all sections

d.  When ready, email document as an attachment to your principal/supervisor

2.  Principal/supervisor:

a.  Review

b.  Type in recommendation, signature and date,

c.  Forward to Superintendent

3.  Superintendent will review, type in recommendation, signature and date, and forward to President of the Foundation. The Foundation will act on requests during their December and May Board Meetings.
