Instructions

Type the information in all applicable sections. Include specific activities and action steps that align to and support the implementation of the associated element. Applicant may include multiple activities that support each element. Please respond to the areas below using single-spaced, 12 point Arial font, and one-inch margins. The page limit for this form is 15-pages (excluding the instructions page). Complete the form by including the timeline to complete each activity and action step, including beginning and ending dates, using both month and year designations, position or persons responsible for ensuring that each action step is completed according to the proposed timeline, and the description of evidence that will be submitted to the California Department of Education upon request for monitoring purposes. Upon submission of the initial PCSGP application, leave the percentage completed column blank. This column is reserved for the reporting of an annual PCSGP Work Plan Status Report. If multiple activities are included within a category, ensure that the start and end dates, responsible person, and evidence narrative all align horizontally with the first line of narrative in the “Element” column.

Enter the percentage completion of all activities for the respective element. Submit the PCSGP Form 7 to the CDE annually along with the annual PCSGP Work Plan Status Report.

Evidence

Type an explanation of measureable evidence that can be used to validate the completion of listed activities. Ensure evidence is measurable, such as the production of meeting notes, the posting of a document, or the posting of web page content.
