Booking Form for Course of Swimming Lessons - Autumn Term 2016

Please note that at the start of this term we’ll organise classes from scratch based on test results from the end of the previous term, thus could you please give us your preferred choice of both day and time for Swimming Lessons for the new term.

Please enclose Total Payment (Twocheques or postal orders , payable to Rusalka Swimming Ltd ) First Chequefor £ 77.00 payment for lessons for the first half term ( 7 weeks) and Second Cheque Post Dated to 17th October 2016for £ 77.00payment for lessons for the second half term ( 7 weeks) and either post or place the bottom part of this booking form, along with your payment, in the black post box in the hall before Saturday 23rdJuly 2016( in order to confirm and guarantee his/her place in the Autumn term) . Any second or third child from the same family shall be entitled to 10% discount on these fees.

You can pay us by a Bank Transfer, however we can only accept payment for the whole term via this method, namely£154.00 paid by 23/08/16. I’m afraid no place will be offered for a part payment . Your first cheque will not be cashed until 22nd August 2016( 2 weeks prior to the start of the term).

On receipt of your completed form and correct payment we will do our best to accommodate your day and time preference and notify you accordingly before 28th August 2016( by e-mail ). In the event that the only placings available are not acceptable to you, then your cheque will be returned or destroyed accordingly.

If, for any reason, you do not wish to carry on with swimming lessons this term could you please let us know a.s.a.p.either by e-mail or verbally to your teacher to prevent us holding the space open for your child unnecessarily.
