NAME OF DEALERSHIPDATE

You have stated that you intend to purchase the following used Toyota vehicle from the above dealership:

MODELYEARVIN

[FOR VEHICLES SUBJECT TO THE FLOOR MAT RECALL]

This document will confirm that you have been informed that the above used vehicle is subject to a recall by Toyota Motor Sales, U.S.A. The recall is intended to reduce the risk of pedal entrapment by incorrect or out of place accessory floor mats. The recall concerns optional All Weather Floor Mats, constructed from heavy-duty rubber, specifically designed for the driver's seating position. If the optional All Weather Floor Mat (either by itself or if it is placed on top of the existing carpeted floor mat) is not secured by the retaining hooks and the mat moves forward, it may interfere with the accelerator pedal returning to the idle position.

[FOR VEHICLES SUBJECT TO THE STICKING ACCELERATOR RECALL]

This document will confirm that you have been informed that the above used vehicle is subject to a recall by Toyota Motor Sales, U.S.A. The recall is due to certain accelerator pedal mechanisms that may mechanically stick in a partially depressed position or return slowly to the idle position. The condition can occur when the pedal mechanism becomes worn and, in certain conditions, the accelerator pedal may become harder to depress, slower to return or, in the worst case, stuck in a partially depressed position.Toyota has advised drivers that if this condition occurs the vehicle can be controlled with firm and steady application of the brakes. The vehicle should be driven to the nearest safe location, the engine shut off and a Toyota dealer contacted for assistance.

[FOR VEHICLES SUBJECT TO BOTH RECALLS]

This document will confirm that you have been informed that the above used vehicle is subject to two separate recalls by Toyota Motor Sales, U.S.A. One recall is intended to reduce the risk of pedal entrapment by incorrect or out of place accessory floor mats.

A separate recall is due to certain accelerator pedal mechanisms that may mechanically stick in a partially depressed position or return slowly to the idle position.

We are giving you this notice because we feel that it is important for you to know that the above referenced vehicle is subject to a recall. By signing this document you acknowledge that you have been informed of the recall and have voluntarily decided to purchase the vehicle. This notice is not intended to create an express or implied warranty, including an implied warranty of merchantability or a warranty of fitness for a particular purpose.
