Hours:37.5 hours per week

JOB PURPOSE:

The post holder is responsible for assisting patients and families to find appropriate 24 hour care from hospital. The aim of the role is to make this transition as smooth as possible for the patient, their relatives and all members of the multi-disciplinary team.

DUTIES AND RESPONSIBILITIES:

The post holder is responsible for:

  1. Communicating with patients and their families once a multi-disciplinary decision has been agreed that 24 hour care is required, either residential or nursing.
  2. Dealing with staff,patients and relatives in a caring, understanding and courteous manner, using tactand diplomacy.
  3. Ensuring that all protocols and policies are followed regarding data protection and confidentiality.
  4. Ensuring all patient correspondence meets Trusts standards.
  5. Taking a proactive approach to communicating with families at the earliest possible date to ensure a patient is discharged as soon as possible after they are deemed medically fit.
  6. Monitor and maintain waiting lists for Nursing & Residential placements, ensuring staff have clear updates. Likewise, your threshold of autonomy should be clear and the mechanism foridentifying and/or expediting patients up to your manager should be explicit.
  7. To develop excellent relationships with all the local home managers.
  8. To be aware of the differences in how each local CCG (primarily Manchester, Trafford and Stockport) applies the legislation around Continuing Health Care, and such processes as the Joint working Agreement and assessment beds.
  9. Participation in audits will be an essential part ofthe role, highlighting trends and pro-active problem solving as necessary.
  10. Take minutes as and when required for Best Interest Meetings, Safeguarding’s and Team meetings in support of the Admin Officer Manager (AOM)
  11. Responsible for the safe and secure storage of all records reporting breaches in regulations and laws to senior managers as appropriate in compliance with Caldicott, Freedom of Information and Data Protection.
  12. Responsible for implementation of administration systems and procedures for the service, to ensure all vacancies lists are regularly updated.
  13. Responsible for identifying priorities and planning own workload.
  14. The receipt and filing of general and sensitive correspondence and information in a proactive manner and ensuring that information is prioritised and dealt with accordingly
  15. Attendance at training events that may be relevant in the execution of your duties, including Mandatory Training in accordance with the appropriate timescales required.
  16. Regularly review administrative processes within the department to continually improve service provision.
  17. Ensure effective communication within the office and across other services where required.
  18. To provide Admin support to Manchester & Trafford Social Services in support of the AOM & absence of junior staff.
  19. To provide secretarial support to both in house Social Work Team & UHSM Leaders returns as and when required in support of the AOM.
  20. Organise & oversee daily Delayed Transfer of Care meetings & returns as and when required in support of the AOM

PARTNERSHIP WORKING

  1. Liaison with medical, nursing and company staff at a variety of locations within the Trust via the most appropriate media.
  2. Liaison with outside agencies.

ANALYSIS and DATA MANAGEMENT

  1. Manage the day to day operation of the Homefinder Service, reporting to the Clinical Lead as required
  2. Maintain patient confidentiality and adhere to the Data Protection guidelines at all times.
  3. Responsible for imputing service performance information to including Trust wide and service specific data
  4. Participate in the implementation of new Trust-wide IT systems and modernisation processes.
  5. Effective use of all electronic systems in use by the service.

EFFORT AND ENVIRONMENT FACTORS

Physical Effort - Combination of sitting, walking and standing. Movement of case notes,and stationary.

Mental Effort – Concentration required when in-putting patient data and communicating with relatives and MDT members. Frequentinterruptions occur.

Emotional Effort – Occasionally contact with anxious or distressed patients and relatives. Communicating with emotionally distressed individual.

Working Conditions – Occasional exposure to verbal aggression.
