- Educators hired on or after October 9, 2017will be placed on Letters of Agreement and will not be formally evaluated during the 2017 – 2018 school term.

- School guidance counselors will be informed of his/her assistance team members
Initial/Preliminary Formative Evaluation Cycle
September 23, 2017– December 9, 2017
September 8, 2017 / PD 5 Consultation Survey Forms (School Guidance Counselors)
- School Guidance Counselor begins distributing Consultation Survey forms
September 28, 2017 / PD 1Long-Range Plan (Guidance Counselors)
- Guidance Counselor submits Long-Range Plan to the team chair
No later than November 10, 2017 / PDs 2, 3, and 6Interviews for Guidance Counselors (Interview Forms)
- Interviews conducted by evaluation team members
November 10, 2017 / PD 5 Consultation Summary Report
- Guidance Counselor submits a copy of the Consultation Summary to the team chair(All documentation for completing the Consultation Summary must be made available to the evaluation team members)
November 10, 2017 / PD7Professional Performance Description
- Building Administrator and/or other designee submits Professional Performance Description to the team chair
November 10, 2017 / PD7 Professional Self-Assessment
- Guidance Counselor submits Professional Self-Report to the team chair
December 11, 2017 / Evaluation Summary/Document Verification Meeting
- Teams must hold consensus meetings on or before December 7, 2017.
December 12, 2017 / Evaluation Summary/Preliminary Conference
- Teams must hold preliminary consensus meetings on or before December 8, 2017.
December 13, 2017 / All preliminary information (Evidence Documentation pp 2-7 and the Evaluation Summary Sheet) and the improvement plan if needed) must be delivered to Human Resources by 5:00 pm
PD 4 –Guidance Counselor reflections must be submitted to the team chair within seven (7) calendar days following each evaluation.
Final Formative Evaluation Cycle
