The Current Health Check contract is commissioned from 1st October 2014 with a contract value of £750,000 per annum. Weight Management contract started 1/4/15 - Leicestershire value - £380k per annum. Stop Smoking Services contract started 1/4/15 - value year 1 Leicestershire £827,281

  1. How long is the current contract for and when is it due to expire?

The current contract runs from 1st October 2014 until 31st March 2016.Weight Management: 2 year contract with 3 year extension period - 31/3/17 expiry date without the extension. Stop Smoking Services: 3 year contract with 2 year extension, expiry date without extension 31/3/18.

  1. Do you have any commissioning plans for health checks and lifestyle services, if so please provide a copy?

Our commissioning intentions are to reprocure the services following the expiry period of each
