We are so excited to welcome you and your child to First UMC Preschool’s 2016-2017 school year!
Enclosed is a packet of forms to be completed before school starts as well as a school year calendar for you to keep. You may mail the forms to the address above or bring them to the Church office any business day. You will receive a hard copy of the Parent Handbook at Orientation/Open House.
You may also download these forms as well as the Parent Handbook. Visit us at www.fumcw.com and click on “Preschool.” An ongoing online Google calendar as well as other information is on our web site!
If you are on Facebook, you are invited to join our closed group, FUMCW Preschool. Please send a request to join and mention your relationship to the preschool.
The “Forms to be Returned” include:
· Parent/Guardian Agreement
· Authorization for Care
· Children’s Medical Report
· All About Me form so that we can get to know your child
· A current copy of Immunization History from your child’s physician
