Instructions: Please READ CAREFULLY and COMPLETE THE bottom SECTION of this form and mail it to the address below with a nonrefundable $5 check made payable to First Presbyterian Church. If the form is not completed in full, it will delay your application process. Registration applications are due Tuesday, September 10, 2013 or will close with the first 50 registrants.
After we receive your application, we will mail/email you the Consignor Packet including all details such as your consignor number, the inventory sheet, sale tags, consignor check-in times, designated pickup time, etc. If you have been a past consignor and would like to request the same consignor number as a past sale please make a note on the bottom of this application requesting your previous number. You must do this EARLY or we cannot promise the number will be available.
Also, we are going to do a “half off” sale for the item you designate to be half off. If you are using old tags, you will need to add this option to your OLD tags. There is a place for this option on the new tags.
A minimum of 20 items must be entered in the sale to participate as a consignor. Persons contributing 20+ acceptable items will receive ONE ticket & 2 guests tickets to the Pre-Sale, Friday, Sept 13th. The consignment sale schedule is as follows:
