1.04Performance and Payment Bonds

Bidder is instructed to carefully review Bond requirements in Section 00300, Bid Security and Section 00611, Performance and Payment Bond. A Bid Bond or a certified check in the amount of not less than five percent (5%) of the bid amount is required at bid opening.

1.05Preparation and Submission of Bid

B.Changes in the bid amount appearing on the outside of the bid envelope will not be considered. Only the bid amount indicated on the Proposal Form inside the envelope will be considered. All changes, corrections and erasures must be initialed by the person signing the bid.

C.Provide a copy of your Registration Certificate as proof of current licensure.

  1. The material costs associated with the Fire Alarm System work shall not be based on the DCPS Maintenance Contract pricing. Work shall be bid similar to all other systems, pricing shall be as per current industry pricing at the time of bid. As part of the bid submittal, the contractor shall provide a breakdown of all specific material line item costs for the fire alarm system equipment as specified below. The bidder shall be responsible for providing a separate document indicating these costs, as an attachment to the bid form.

1.06Mandatory Pre-Bid Conference

All contractors who are interested in bidding are required to attend a mandatory pre-bid conference to be held onJanuary 18, 2012 at9:30 amlocal timeat J.E.B. Stuart Middle School No. 207, 4815 Wesconnett Blvd., Jacksonville, FL 32210. Failure to attend the pre-bid conference shall result in disqualification of that firm’s proposal. Attendees will be required to sign an attendance register.

1.07Notice of Direct Purchase - Sales Tax Exempt

  1. The Owner is exempt from sales tax on the purchase of construction material. The Owner has elected to exercise this right and therefore directly purchase various construction material, supplies and equipment that may be a part of this Contract. Such direct purchase shall be without any additional cost to Owner. The Owner shall, via Purchase Order (PO), purchase material, and the Contractor shall assist the Owner in the preparation of the Purchase Order. The Owner will purchase the material from Vendors selected by the Contractor for the price originally negotiated by the Contractor. All bids are to be submitted with all applicable taxes included.
  1. The Contract Amount shall be reduced by the net, undiscounted amount of the purchase orders plus all State sales tax. This reduction in the Contract Amount will occur through a Change Order, which will reference the Purchase Order affecting the change. OEO Form 6 is also required.
  1. Issuance of Purchase Orders by the Owner shall not relieve the Contractor of any responsibly regarding material or equipment purchases or installations, with the exception of the payments for the material or equipment purchased. The Contractor shall remain fully responsible for coordinating, scheduling, ordering correct quantities, submittals, protections, storage, shipping, security, expediting, receiving, verification, installation, cleaning and all applicable warranties. The Contractor must maintain the Builder's Risk policy to include all material and equipment stored on-site and installed on site.
  1. It is recognized that the Contractor may encounter additional overhead costs in assisting the Owner with its Direct Purchase Program. The Contractor is charged with including all additional costs as part of the Base Bid.
  1. No payment will be made for material or equipment stored off-site.

1.08Prequalification Requirements:

  1. All contractors submitting proposals and bids must be prequalified with Duval County Public Schools at the time of the ITB Response Due Date. No proposals or bids will be accepted from Contractors who are not prequalified at that time. B.Contractor Professional Experience

Contractors must provide the following information in their application for review by DCPS. Applications not providing this information shall be rejected.

1.Indicate total years of company experience in the Northeast Florida area.

2.Contractor shall have been in business for at least five (5) years.

3.Contractor shall have installed at least three (3) projects of similar size and complexity.

4.Not used.

5.Describe prior experience (if any) in performing work for DCPS.

6.Indicate any factors which would prevent the Contractor from working in all DCPS buildings at all locations within the geographical boundaries of DuvalCounty.

7.Specify the number of Contractor technicians who will be available to perform work and total years of experience, resumes, specialized training of each technician.

8.Describe the nature and scope of any other technical services the Contractor is qualified to perform.

D.Other Contractor Requirements

1.Upon request for DCPS or its representatives, the Contractor will provide the names, addresses, social security number and employment history for those technicians who will be performing DCPS work for verification of experience and for background checking, if necessary.

2.The DCPS will require that while on DCPS property that the Contractor’s technicians dress in a manner appropriate to the work to be performed. Contractor technicians will perform their work in a courteous and efficient manner.

3.Contractor must provide a visible employee badge, or other means of identification, that must be worn at all times by each technician while on DCPS property.

4.The Contractor will secure, pay for, and comply with all permits and certificates that may be required by local, state and federal laws and ordinances.

5.Contractor must provide their technicians with transportation, tools, and equipment appropriate to the work to be performed. The DCPS will not provide transportation and will not furnish tools and equipment.

E.Contractor Services to be provided

This paragraph outlines the services to be provided by the applicant.

1.Operations Requirements

a.Contractor must complete all DCPS work in full conformance with the most current applicable standards for electrical and low voltage wiring.

2.Contractor must perform all required functional testing of system wiring.

3.Contractor must reinstall, restore or reconnect any and all equipment, devices, or fixtures which may have been moved or temporarily disconnected as a result of authorized system installations, modifications, adds, moves and changes or other work. Upon completion of the work, Contractor personnel will clean up and restore the work place and associated equipment rooms to their original condition. When any work is being performed in occupied areas the work site must be cleaned daily.
