FINGERPRINTING PROCEDURE FOR VOLUNTEERS

Who is required to be fingerprinted?

District policy states that parent volunteers who are taking students off campus alone (e.g. driving students on a field trip) must be fingerprinted. Volunteers who chaperone for the Coloma Outdoor Discovery Program MUST also be fingerprinted. If other field trip venues require chaperones to be fingerprinted, we will let you know.

Do I have to be fingerprinted every year?

No, this is a one-time requirement & FINGERPRINTING IS VALID THROUGH YOUR CHILD’S 8th GRADE YEAR.

What is the process to get fingerprinted?

1)  Pick up the required ‘Request for Live Scan Service Form’ from the office

2)  Arrange for fingerprinting at a location that offers Live Scan services, such as the following agencies:

San Mateo County Office of Education

ü  Call 650-802-5300 or email SMCOE at to set up an appointment

ü  Indicate that you are a parent volunteer for Brewer Island Elementary (SMFC District)

ü  Indicate your preferred day and time slot from the schedule below:

M – F (8:30-9:30 am) (10:30-11:40 am) (2:00-2:40 pm) (3:30-4:20 pm)

ü  Take the following items to your appointment:

a) ‘Request for Live Scan Service’ Form

b) Valid photo ID (original Driver's License, Passport, or State ID Card)

c) Cashier’s check or money order only (no cash, please) in the amount of $44

ü  The SMCOE is located at 101 Twin Dolphin Drive, Redwood City, CA 94065

Police Stations, UPS Stores and other Agencies (see reverse side for some local listings)

ü Call the agency to set up an appointment and confirm pricing & payment form accepted

ü Take the ‘Request for Live Scan Service’ Form, valid photo ID & payment to appointment

3)  After fingerprinting is completed, return the yellow copy of the ‘Request for Live Scan Service’ form to the school office with your child’s name and grade written on the form

4)  The office will notify the teacher when fingerprinting has been cleared

5)  If your fingerprints do not clear, you will be notified by the SMFC District office.
