Final ReportGuidelines and Form

As indicated in your grant award letter, The Asian Giving Circle, an initiative of The Chicago Community Trust, requires your organization to submit a Final Report including narrative and financial information on activities related to the grant. The Trust emphasizes the importance of Final Reports, as they are an integral part of our evaluation process. They are not only necessary for proper oversight to ensure accountability, but also serve as a valuable learning tool for the Asian Giving Circle, Trust staff and your organization. We greatly appreciate your candor and thoughtful review of your project/program experience. Be assured that we do not share these documents with any organizations or funders.

GUIDELINES

  • Final Reports include a Narrative and a Financial Report, and are due on or before the due date. Once reports become past due, applications currently submitted and/or future funding considerations will be affected.
  • Final Reports are an indication that your project/program is complete and that all grant funds have been spent. If you received funding for a strategic plan, please provide an update on the timeframe indicated in the proposed grant application, progress towards implementing the plans and report if any grant funds remain unspent at the time the Final Report is due.
  • NarrativeReport:
  • Answer all questions in the report. You may recreate the questions on your own word document.
  • The final narrative report should present a review covering the entire grant period.
  • Financial Report:
  • Please attach a copy of your agency or program budget that was submitted with your original application along with actuals-to-date (explain any variances).
