Meeting Objectives:

Attendees will understand the many steps involved in designing, implementing and spreading an effective program to reduce preventable venous thromboembolic events in line with the goal of the Partnership for Patients to reduce preventable patient harm by 40%.

  1. Attendees will understand the requirements for assembling an effective project team focused on reducing patient harm from VTE.
  2. Attendees will become familiar with evidence based protocols known to be effective in reducing patient harm from VTE.
  3. Attendees will understand metrics demonstrating the successful implementation of interventions to reduce patient harm from VTE.
  4. Attendees will be able to identify specific tests of change proven effective in rapid cycle process improvement focused on reducing patient harm from VTE.

Presenters/Panelists

Dale Bratzler, DO, MPH

Dale Bratzler, DO, MPH, is a Professor in the Department of Health Administration and Policy, and Associate Dean in the College of Public Health, and a Professor of Medicine in the College of Medicine at the University of Oklahoma Health Sciences Center. He has recently assumed the role of Chief Quality Officer for the OU Physicians Group. Dr. Bratzler has worked in the field of healthcare quality measurement and improvement for many years. He served in various roles including medical director and chief executive officer for the Oklahoma Foundation for Medical Quality over a period of nearly 20 years. For many years he has worked on the development and maintenance of national performance measures used to profile and publicly report metrics on quality of inpatient and outpatient healthcare as a contractor to the Centers for Medicare & Medicaid Services, and has served on many committees or technical expert panels related to healthcare quality including three years as co-chair of the Technical Advisory Panel for the National Quality Forum DVT Prevention & Treatment Project.

Dr. Bratzler recently completed his second stint as President of the American Health Quality Association, is a recent member of the National Advisory Council for the Agency for Healthcare Research and Quality (AHRQ), and is currently a member of the Healthcare Infection Control Practices Advisory Committee (HICPAC) for the Centers for Disease Control and Prevention. He has published extensively on issues related to evidence-based management of hospitalized patients and frequently presents locally and nationally on topics related to healthcare quality, particularly associated with improving care for pneumonia, increasing vaccination rates, and reducing surgical complications. His background includes both private and university-based practice of general internal medicine.

Denise Remus, Ph.D., RN

Dr. Denise Remus is an Improvement Advisor with Cynosure Health. With more than 30 years of health care experience, she is considered an expert in health care quality measurement and improvement, pay for performance, and risk and severity adjustment. Prior to November 2011, she served as the Chief Quality Officer for BayCare Health System, the largest, community-based health system in the Tampa Bay region. For five years, she was responsible for overseeing BayCare’s quality programs and the health system’s performance improvement and patient safety initiatives.

Other previous roles include Vice President of Clinical Informatics for Premier, Inc. and Senior Research Scientist for Quality Indicators at the Agency for Healthcare Research and Quality (AHRQ), an arm of the United States Department of Health and Human Services. She has served on numerous national committees and technical advisory panels for organizations such as the National Quality Forum, the Medicare Payment Advisory Commission (MedPAC), Alliance for Cardiac Care Excellence, American Nurses Association, Robert Wood Johnson Foundation, and the National Association of Health Data Organizations.She has practiced in a variety of clinical areas including medical and surgical nursing, critical care, hospice home care, long term care, and as a geriatric clinical nurse specialist. Dr. Remus began her career as a commissioned officer and nurse in the United States Air Force.

Karen Gessner

Ms. Gessner is the Quality Coordinator at Valley Regional Hospital. She has been an RN at the bedside for 30 years, the last 20 at Valley Regional Hospital. Starting in the ICU and moving to OB after 2 years. In the summer of 2011, she moved to the Quality department as assistant to Sandy Gee CPHQ. Karen is preparing to qualify as a Certified Quality Professional in the next year.

Kathy Helberg

Ms. Helberg has worked at SNHMC for 26 years; as a Quality and SafetyCoordinator since 1990 and prior to that as a Nurse Manager for amedical surgical orthopedic unit. She is a Certified Professional inHealth Care Quality and holds a BS in Nursing at Adelphi University. She

started her career as a CCU nurse on Long Island, NY. Her focus isoverall coordination of quality assessment and improvement activitiesincluding the evaluation and coordination of measurement and assessmentof performance improvement activities in Surgical Services, Cardiac CathLab and Invasive Radiology areas. She also provides facilitation support

for the Surgical Care Council, Endoscopy Committee and AngioplastyCommittee.

Lynda Pittenger

Lynda Pittenger has worked as a nurse at SNHMC for over 25 years and hasbeen in the Quality and Safety Department since its inception in 1992.She is a certified professional in healthcare quality. Her focus hasbeen related to improving processes in the medical arena related to

clinical excellence, patient satisfaction and safety. She facilitatesthe P&T Committee, the Cancer Committee, the Stroke Team and the MedicalCare Council.

Paul Santos, Pharm. D.

Paul is currently the Clinical Coordinator for the Department of Pharmacy at Lakes Region General Hospital.He received his Bachelor of Science in Pharmacy degree from Massachusetts College of Pharmacy in 1983.In 1989 he received his Doctorate in Pharmacy from the State University of New York at Buffalo.For the past nineteen years Dr. Santos has been involved in clinical practice/research with anti-infective therapy. Research has involved clinical and in-vitro studies.
