This information is needed because awards are cash awards and therefore subject to taxation. If you receive an award, the Finance Department of the Santa Barbara Foundation will be sending you a form 1099 in the Fall of 2009 as required by law. You will need this form in completing your or your parents’ 2009 taxes.

We understand you may have concerns regarding the security of giving us your social security number. This form will be shredded immediately after awards are made in July. Please be assured that we take great care in keeping all application materials secure.

In order to make it easy for us to shred this one page, make sure your social security number only appears on this one sheet of the application.
