FALMOUTH MIDDLE SCHOOL NOOK BORROWING AGREEMENT

Parent Responsibilities and Permission:

  • I am authorizing my child, ______, to take out a NOOK e-reader from the FMS Library. We understand that it is a tool for reading and learning.
  • My child will comply with the Falmouth Middle School Acceptable Use Policy found in the school handbook, I will help to ensure the safe and timely return of the NOOK within the loan period of two weeks.
  • I also understand that I am financially responsible for any loss of the device or willful, or accidental damage to the NOOK, as well as any charges that result from content downloaded without written permission from the FMS Librarians (Mrs. Becksvoort or Mrs. Nichols).
  • I understand that my child may lose future borrowing privileges of the NOOK if there is damage or if the device is not returned in a timely manner.

  • I agree to take care of the FMS Library NOOK while it is in my possession. I will not lend or let anyone else use the NOOK to anyone while it is signed out to me.
  • I will take care when putting into my backpack that it is not on the bottom or where it can become damaged.
  • I will use the NOOK for its intended use - reading ebooks. Books may be borrowed from the OverDrive library - ask Mrs. Becksvoort for help with this.
  • I will not download any content from the internet without written permission from Mrs. Becksvoort or Mrs. Nichols.
  • I agree to return the NOOK in good condition within the two week loan period.
