Girls’ Volleyball 4th – 8th Grade

Dear Parents and Athletes,

Your son/daughter has showed an interest in participating on one of our Saint Anne School Fall Sports team.

Players’ Responsibilities:

• Maintain a “C” (70%) average in all classes, and complete class and homework assignments.
• Follow school’s handbook regulations (e.g. behavior, uniform, gum, walking home, daycare, shoes, and socks) and the Athletic Handbook.
• Attend all practices, they are mandatory. If you cannot attend practice, you must notify your coach before practice begins.

*Boys’ Practice - 3:20 - 4:30 Monday - Thursday

*Girls’ Practice - 3:20 - 4:30 Monday - Thursday

• Return this form, the emergency permission form and the Athletic Contract from the Athletic Handbook with the athlete’s and parents’ signature on each of the forms. These forms must be returned to Mrs. Price by Wednesday, September 2nd, 2015. Football & volleyball practice begins Tuesday, September 8th, 2015.

Parents’ Responsibilities:

• Pick up your child immediately after practice/games, if not, they will be sent to Afterschool Extended Day Care, and charged accordingly. There is a 15 minute grace period.

• If your child/children walk/s home, he/she must have a letter stating that you have given your permission for them to walk home.
• Drivers for games are only allowed to drive to the game location and back to St. Anne. Not anywhere else, this is for liability purposes.
• You are required to pay the sports fee before September 16th, 2015 unless you have spoken with Sister Teresa to make arrangements.
• You are required to drive to at least one game or drive your child to the games. (The requirement to drive is stated in the Athletic Handbook)

Cost: (Sports Fee is due by September 16th)

• Fee is $120.00 that includes rental of their uniform.
• Spirit T-shirts are $20.00. This is a mandatory fundraiser. If you have a Spirit T-shirt from last year, you do not need to purchase one. Spirit T-shirts are worn on Spirit Days and certain game days with blue jeans to promote school spirit. Spirit shirts are to be worn over their jersey. You may pick up a Spirit T-shirt Order Form from the Art Room or the office.
• Payments are due to Mrs. Price. (Do not leave any monies or check outside of the Art Room) If Mrs. Price is not in her room, payment should be delivered to the office.
