Please complete this form for EACH soccer player in the Fairview Aggie Soccer Program. It is VERY IMPORTANT that you list the cell number where you will get informational texts in the cell # blank. We will be utilizing Remind 101 along with emails and other possible forms of electronic communication throughout the year. PLAYERS: It is YOUR responsibility to keep parents/guardians informed about all information given to you by your coaches. We will also have a “Team Parent” for each team that will do their best to send out informational texts or emails.

Membership Dues and Player Costs

Player Fees include: full uniform (except for cleats & Shin guards), field maintenance, tournament fees, pre-game snack for away games, team equipment and booster club membership dues. Additional warm-ups (optional) will be available for purchase for an estimated $75 per player.

FEES: $200.00 per player – a portion of your ad sales will be applied to your fees. There will be several fundraisers before the season begins that may be applied to your fees. Part of this $200 is the Activity Fee. A $50 Activity Fee is due by November 6th, 2015.
