Teach Back Practice: An Exercise Plan

•  Triad Role Play Practice: Patient, Clinician, and Observer

•  Clinician: Your patient tells you she likes to walk and can commit to walking around the block three times a week at minimum, and keeping a record of the exercise.

•  Clinician: Practice Teach-Back in the context of relationship-based care

•  Patient: Do your best to follow along. Initially omit the record-keeping piece of the plan

•  Clinician: Continue Teach-Back as needed

•  Observer: Share strengths and opportunities to stretch

•  Shift roles and repeat.
