Excellent HSE Performance
RATIONALE & GOAL:
This award will recognize successful business models from organizations that uphold HSE as a key business value, and excel in both HSE and business performance.
The award aims to:
- Recognize entities that have HSE well integrated as a key organizational value and in which measurable achievement in HSE performance is linked to productivity and profitability.
- Establish a validated process by which companies can measure the performance of their HSE systems against well-tested and internationally accepted systems.
- Through a rigorous systematic review process, capture and evaluate successes and as applicable, share HSE lessons learned.
- Foster the sharing of leading HSE management systems and best practices for educational purposes.
SELECTION CRITERIA:
Business Profile
Briefly describe the type of business, principal products and/or services, size of workforce, including number of employees, contractors, and temporary workers, annual sales volume, and other relevant information. The business profile should include evidence that the organization operates as a financially viable entity.

Section 1: Results
Please provide information on the following:
2018 YTD / 2017 / 2016 / 2015- Lost Time Accidents
- No. of lost time accidents (LTA) i.e. accidents with human injury which resulted in days away from work (not counting the day of the accident) as certified by a registered Medical Doctor.
- No. of persons injured (for all the LTAs in the respective year).
- No. of days lost (for all the LTAs in the respective year).
- No. of cases of work-related ill-health or disease.

- No. of Administrative Orders under the Environmental Management Act Chapter 35:05

Section 2: Leadership
Describe how the leadership of the organization demonstrates its commitment to upholding HSE as a key business value as outlined by the goals of the awards scheme. Discuss the strategies, programs and methods used to build management’s level of commitment to HSE goals, corporate culture and your organization’s HSE impact on the extended/global corporate community, and other pertinent factors.

Section 3: HSE Systems
3.1Policy Management Commitment
- Does your organization have a documented E/OSH Policy? If so, provide a copy;
3.2Planning
Identification of environmental impacts, OSH risk assessment, planning for legal compliance, structuring HSE planning resetting objectives, targets, programs, etc.
- Does your organization have a formal process for OSH risk assessment, and if so, how are the findings documented? (Provide a sample page.)
- Does your organization have a formal process for identifying those aspects of your operations that can adversely impact on the environment, and if so, how are the findings documented? (Provide a sample page.)
- What are the key job positions with responsibilities in the planning process referred to in (a) and (b) above?
3.3Organisational Structuring for HSE
- Provide a copy of your organization chart, and identify the positions with key E/OSH responsibilities; also indicate the size of the workforce. (i.e. permanent, casual or temporary and contractor employees)
3.4Training and Awareness
- Provide a copy of your training plan related to HSE for the years 2017, 2016 and 2015;

3.5Communication (with Internal, External Parties)
- Outline the strategies, programs, projects and methods used to communicate HSE with employees and external stakeholders.
3.6Stakeholder Relations (Internal, External Parties)
- Outline the company’s primary HSE activities and projects with employees and external stakeholders.

3.7Control of Operations at the Company or Facility
- Has your organization identified those operations/activities/jobs in your workplace that can present significant OSH risks, or that have the potential to adversely impact on the environment? Are these documented? State in general/outline the types of controls in place to eliminate, minimize, control, mitigate or manage these adverse OSH/E risks.
3.8Management of Contractors
- Outline key aspects of your company’s system for managing contractors for HSE.

3.9Emergency Preparedness and Response
- Has your organization conducted emergency drills in the past 12 months? If so, when? Does your company have documented emergency preparedness and response plans?
3.10Performance Measurement and Monitoring
- List the main proactive (or leading) KPIs that your organization uses to assess OSH performance, and E performance, or both if integrated.

3.11Incident Investigations
Handling accidents, incidents and other non-conformities, including how corrective and preventive actions were identified, developed and/or implemented.
- Does your company have a documented procedure for Incident Investigation and handling OSH/E nonconformities? List or outline the main tools or methods used by your organization to investigate incidents and accidents, and to identify root causes, and corrective and preventive actions. (Tools/methods may include: Ishikawa Diagram, 6Why’s, etc.) What are the key job positions with responsibilities for incident investigations?
3.12Record Keeping
- How do you assess the effectiveness of your record keeping?

3.13Auditing of the OSH/E Management System
- When last was your OSH/E Management system audited internally, and externally? Provide a copy of the recent Audit Schedule for internal and for external audits.
3.14Management Review of OSH/E Management system.
- Does your Management conduct a comprehensive review of the entire HSE system? How frequently? Are records maintained?
3.15Health and Wellness
- Outline programs and initiatives in place to address employee health and wellness and the percentage of employees involved in health and wellness initiatives
