EVALUATION OUTLINE

WHAT ARE WE TRYING TO ACHIEVE? The overall aim of the national falls programme is to reduce harm from falls. The Ask, act, assess process, tool and resources contribute to this aim by

1  Encouraging staff to raise questions about falls risk with older patients in order to identify those who require a fuller risk assessment

2  Providing guidance for staff by outlining risk factors for assessment, linked to actions to take when risk factors are identified - including referral or handover about risk factors, or patient/family education with the Patient Letter and ACC Home Safety Checklist.

YOUR FEEDBACK Please arrange for staff input to this evaluation e.g. collect their comments on this form and/or through focus group. It would be great to have your honest responses on how the screening and referral/handover process worked, i.e. did it actually help find patients at risk of falling? How easy/difficult was it to talk with them? Or arrange for follow-through on identified fall history and risks with referral or handover? Please use the comments column below, or a separate page.
