Activities (July 1 - December 31, 2014)

For questions 1 & 2 , you may complete a “Progress to Date” column on your SOW chart in place of these questions.

  1. Please summarize progress toward each goal area in your Scope of Work (list each goal and summarize activities completed under each goal during the reporting period). Where possible, please include appropriate dates, numbers (e.g., clients served, units of service provided), and a description of any deliverables created.
  1. Please provide a description and explanation of planned activities that are behind schedule. Include suggested resolutions for delays you have experienced.

Required Attachments

For EQuIP partners serving parents/families (please note changes below effective this reporting period):

  • Submit original, completed First 5 Parent Surveys administered between 7/1/13 – 12/31/14. Forms and surveys should be batched together by service or workshop, and each batch should include the First 5 San Mateo Data Submission Cover Sheet on the front, describing who the form/survey was administered to and when.
  • For Raising a Reader: Submit the First 5 San Mateo Aggregate Data Collection Tool, providing totals for all parents served across workshops provided between July-December, 2014.

For EQuIP partners serving ECE providers and students: Submit original, completed First 5 Early Childhood Education (ECE) Provider Survey Forms administered between 7/1/14 – 12/31/14. Forms and surveys should be batched together by service or workshop, and each batch should include the First 5 San Mateo Data Submission Cover Sheet on the front, describing who the form/survey was administered to and when.

Data Entry Reminder

Please remember to complete data entry for SMCCCD activities, RAR provider services, and ECLDI provider services supported with EQuIP funds between July-December, 2014. Data for other services/agencies not mentioned do not need to be entered into the database. Any staff entering data must be trained personally by EQuIP consultant Diana Harlick. Please contact Diana at for more information and/or to schedule a training.
