End of Year Annual Review LPAC

The LPAC meets at the end of each school year to review all pertinent information of students in order to:

1.  Designate the language proficiency level of each LEP student,

·  Current oral language proficiency level as measured by TELPAS or WMLS-Revised (for exiting students)

2.  Designate the level of academic achievement of each LEP student,

·  PK – 2 grades: Grades and CBA’s

·  Grades 3-12: Grade averages, STAAR Results (Reading, Writing, ELA, Mathematics, Science, and Social Studies)

3.  Recommend additional instructional interventions for adequate yearly progress

4.  Review criteria for students meeting exit guidelines

5.  Monitor student progress for academic success of all LEP students

6.  Monitor former LEP students for two years following exit.

·  Grade averages in content areas

·  Pass STAAR Reading in grades 3, 5, 6, 8

·  Pass STAAR Reading/Writing in grades 4 and 7

·  Pass STAAR ELA in grades 9-10

·  Pass TAKS ELA for students 11 -12

7. Report student progress to parents/guardians.

·  Form 20 a(English/Spanish)--For students exiting the program, send two copies of this form home. One is to be signed and returned and placed in folder.

·  Form 20 to notify parents of progress

·  Notify me to change PEIMS status if a student qualified based on Oral proficiency, reading and writing scores

EOY Checklist:

____1. Complete oral language proficiency assessments as needed for progress/exit criteria

____2. Complete End of Year LPAC Forms for each student

·  Form 19 -- Annual Review

·  Form 20 – Parental Report on Student Progress (E/S)--See counselor to coordinate the delivery of this information with the mailing of TAKS and TELPAS results. File a signed, dated copy in the student’s blue LPAC folder as documentation for exit.

·  Form 20(a)—Approval for Exit

·  Form 18 – LPAC EOY Summary Roster—Copies to Principal, Counselor, and Director of Special Programs

____ 4. Complete Monitoring Progress reports for M1 and M2 students

·  Form 22—Monitoring of Reclassified Students

·  Notify me if there are changes on status or if M1/M2 needs to be reasigned

____ 5. Complete end of year summary info

·  Form 23—Title III Data Collection --Turn in one form per grade level.

·  Form 25 – End of Year Summary Roster with LPAC signatures.

____6. NOTIFY ME IF THERE ARE CHANGES IN PROGRAM CRITERIA (EXITING STUDENTS) SO THAT I CAN UPDATE PEIMS
