50N (ST50N, PT50N, OT50N)
This is how you add new goals.
  1. Email Scheduling to get the visit on your device.
  2. Have them schedule it on a day you DON’T see the patient.
  3. Visit does not require you to see patient or to get parent signature.

Tap on the visit with the service code “ST50N.” / Tap “Start.”
Tap “Yes.” / Tap “Continue.”
To complete the Visit Actions, do the following:
Tap “Visit Actions.” / All areas with an asterik will need to be completed.
Physical Assessment
Tap “Physical Assessment.” / The screen will say “There are no items to display.” Tap the back button. Screen will advance back to the visit actions screen.
You will get a green checkmark next to “Physical Assessment.”
Interventions / You will have a green checkmark next to “Interventions.”
Complete as you would for a daily visit, but select “Not Provided” for each goal area and choose “Not Addressed at This Visit” for the reason. /
Tap “Therapy Goals/Status.” / Tap your discipline.
Tap the desired goal category. / Choose the goal you want to edit.
Other Ways to Address Goals:
FOR OTs, PTs, STs ONLY (NOT for COTAs, PTAs, STAs)!
  • To modify a goal: Change goal percentage or cueing level and explain in comments
  • To master a goal: Status should match or exceed goal, tap box next to “Don’t Carryover,” write “Goal mastered” in comments. Our policy is that a goal is mastered when the goal has been reached for 3 consecutive sessions.
  • To discontinue a goal: Make goal match status (bring goal % down), tap box next to “Don’t Carryover,” write “Goal discontinued due to ______” in comments

Comments on goals:
  • If a generic goal is used (Expressive Language 1, etc.): Be sure to copy the actual goal from the comments section of the last visit or the evaluation and paste it in the comments section. If you don’t, no one knows what the goal is.
  • If you make a goal from the goal bank more specific: Put the specifics in parentheses or mark them in some way so they stand out.
  • For all goals: The comments section is where you will make statements about the child’s performance on each goal at each session.

Tap the back button to return to the list of visit actions. You will have a green checkmark next to “Therapy Goals/Status.”
Tap “Sign out.” / Tap “Choose a disposition.”
Tap “Complete.” / Tap “Complete.”
Tap “Agent Signature.” / Sign with your credentials and tap “Save.”
Tap “Client Signature.” / Tap “Caregiver.”
Type, “50N-No signature needed” and tap “Save.” / Tap “Save.”
You will have a green checkmark next to “Agent Signature” and “Client Signature.” Tap the menu button. / Tap “Save.” Database will automatically backup.
