The Target Store on 88-01 Queens Blvd (54th Ave & Queens Blvd.). in Queens, Elmhurst, NY was inspected by Thomas Branham, Elevator Consulting Services, Inc., on May 3, 2015 to assess the equipment reliability and status. My QEI number is 1339 and I am certified by NAESA. NAESA is the National Association of Elevator Safety Authorities. We have identified corrective items that in our opinion need to be addressed by the Elevator Contractor. They are free to offer an explanation or dispute any individual item within the report but we can offer a reasonable reason for each and every negative evaluation. They are evaluated on and for the ASME 17.2 Code for Inspectors Manual and industry standards for acceptable installation and maintenance procedures.

22.  Clean the pit of all oil and oil contaminated absorbents. Replace leaving Victaulic seals as needed. Document the oil loss on the oil monitoring sheet. Check the packing. They appeared fine but not sure if they are the leaking culprit. Recommend the scavenger pump get replaced with a pit bucket of add a check valve in the return line.

Note: Unit shut down waiting on hoist cables. I repeated most everything base on visual condition and disregard for most of the punch list; waiting for the mod to start.

23.  Determine why the door close button does not always override the auto close timer. Install or remove the jumper from studs 15 to 23 accordingly. Make the door close button override the auto close timer. REPEAT REQUEST

24.  Replace the thrust bearing, bad rumble in the gear. It is either the thrust bearing or a motor bearing. Eliminate the rumble. Gear teeth are starting to get a sharp edge, have a lot of pock marks on the lag (rear side) of the gear teeth. REPEAT REQUEST
