Job Title: Roads to Community Living, Program Coordinator

Job Summary: Provides relocation support for individuals moving from institutional settings to community settings as authorized by case management staff in the participant’s Service Plan. The Program Coordinator will also serve as the main intake and administrative support person for the RCL project.

ESSENTIAL FUNCTIONS of the job:

  1. Receiving referrals for Full Life Care’s Roads to Community Living Project, entering referrals into client documentation system and ensuring timely beginning of services;
  2. Completing and submitting month-end reporting process, including compiling timesheets, excel spreadsheets and other documentation required for program funding;
  3. Conducting pre-transition consultation with participant to review RCL Service Authorization, and review participant needs and preferences;
  4. Act as a liaison on behalf of HCS and AA staff, SNF staff, Housing providers, medical and legal providers, family members, informal support and other involved parties;
  5. Providing assistance and support to ensure successful transition from institutional setting as authorized by AAA and/or HCS staff;
  6. Locating and arranging for appropriate, accessible housing;
  7. Working with local housing authorities and other housing resources regarding housing availability, subsidies and voucher capacity;
  8. Arranging of participant’s physical move from institutional setting;
  9. Serving as a liaison between the participant and housing provider;
  10. Accurately tracking and reporting purchases, filing receipts and other activities related to purchases made on behalf of RCL participants;
  11. Assistance with developing personal skills related to the individual’s care plan including:
  12. Helping the participant achieve measurable goals;
  13. Helping the client access health services in a community setting;
  14. Helping the client achieve personal skill development related to Service Plan tasks;
  15. Self-Advocacy training;
  16. Connecting participant with less formal or atypical daily activities such as those that might be found through local organizations like YMCAs or Senior Centers.
  17. Directing other RCL staff at Full Life Care in a team setting to successfully complete participant transitions in a timely manner;
  18. Communicating participant progress with HCS/AAA staff;
  19. Completing reporting requirements such as time sheets, weekly/monthly reports and other documentation required for RCL participants;

MARGINAL FUNCTIONS of the job:

  1. Complete projects and tasks as assigned by supervisor,
  2. The ability to prioritize projects needing to be completed in a timely manner,
  3. Collaborate with and train staff proper operation and maintenance of appliances,
  4. Availability to be reached and respond quickly to facility emergencies,

Qualifications of the job:

  1. Ability to read, write and understand directions;
  2. At least eighteen (18) years of age;
  3. Must have access to private reliable means of transportation;
  4. A minimum of two (2) years work experience in social services or related field;
  5. Ability to work in various settings, including in a team setting with other RCL staff;
  6. Ability to communicate with clients and other stakeholders in RCL project;
  7. Demonstrates the following characteristics:

Educational Requirements Needed to Perform the Duties of the Job:

  1. B.A Degree in Social Work, Psychology or related field;
  2. Ability to effectively use computer and communication technology;
