Submit Child Find/Public Awareness Quarterly Report
Begin work on APR. Analyze data for Indicator 2 (Natural Environments), Indicator 3 (Early Childhood Outcomes), Indicator 4 (Family Surveys), Indicators 5 & 6 (children served by county) and Indicator 7 (45-day timeline for initial IFSP). Begin recording results in APR template and add appropriate improvement activities.
7/20 / All data for Indicators 1, 7, 8a and 8c to be cleaned-up and corrected by July 20, 2012
7/31 / Distribution of pre-populated APR templates should be received by July 31, 2012
Verify all subcontracted employees certification or licensure is current and a DDSD Subcontractor From has been submitted.
Document trainings and reflective supervision on your IPDP and tracking sheet (training specific to self-assessment competency areas).
Schedule review of any staff IPDP that are due for review. Make sure personnel that have left the agency have been changed to Inactive Status in FITKIDS.
In coordination with other providers sharing counties with your agency, assess child find needs and coordinate activities for new fiscal year.
Other: Pre Phase 1 July to September including Orientation/Overview, scheduling of Phase 1 On-site visit and identifying of the agency’s core team, assure all core team members have viewed The Transdisciplinary Team Approach
