JOB DESCRIPTION

Job Title: / Reintegration Broker (Peer Support, Activities and Mutual Aid Champion)
NJC Scale Point: / Pt 22 – 28 (pro-rata) depending on experience

Hours:

/ 37.5 per week
Annual Leave: / 26 days plus statutory and bank holidays (1 Additional day after each year of service, up to a maximum of 31days).
Location: / South Gloucestershire based (or any other location your employer may reasonably specify)
Pension: / Contributory pension scheme (employer’s contribution 7% to a minimum 3% contribution from employee).
Accountable to: / Reintegration Team Leader
Responsible for: / A caseload of up to 40 clients

Principle Purpose of the Job

Working within the Reintegration Team as part of DHI’s Drug & Alcohol Treatment Service the aim of the role is to broker tailored packages
of support that deliver interventions focusing on the completion and
re-integration phases of treatment.

Specifically:

  • To provide the service’s key activities around structured recovery support: group interventions; abstinence and aftercare support; welfare, benefits and housing support; peer networks (specialising in one or more of these areas.); employment/training/education services; community integration and involvement;
  • To act as the Peer Support, Activities and Mutual Aid Champion in the Reintegration team
  • To broker internal and community resources enabling clients
    to implement and sustain positive change and increase recovery capital through playing an active role in their communities

Key duties and responsibilities:

Operational Duties

  • Conduct skilled psychosocial needs assessments, enabling clients to reflect on, and indentify issues central to their eventual recovery (such as: drug use, emotional wellbeing, finances, legal, housing, social functioning etc).
  • Assist clients to formulate Recovery plans, based on their identified needs, by enabling clients to explore a range of support options that will support and enhance their recovery; including appropriate services within
    the Treatment Service and services within the wider community.
  • To broker effective packages of support that meet the varying needs
    of clients at different stages in their recovery journey.
  • To deliver effective floating support for clients with housing needs, including new accommodation, sustaining tenancies, benefits advice, accessing grants, etc.
  • To liaise with, develop and maintain good relations with a wide range
    of third party organisations relevant to the client's support needs including: registered social landlords; private landlords; activity and support groups; colleges and educational establishments.
  • To act as the team's Peer Support Champion including:
  • Acting as 'resident specialist' on peer support, activities and mutual aid keeping up-to-date with new initiatives and good practice.
  • Promotion of peer support initiatives amongst staff and clients.
  • Network, develop relationships and be the main point of contact with relevant external organisations.
  • Support the Reintegration Apprentices to develop (or facilitate the development of) new activities, peer mentoring and support
    and mutual aid options including SMART RECOVERY.
  • Assume responsibility for any particularly complex cases within your specialist area.
  • To assist clients to access the Housing Achievement Programme (HAP)
    in order to prepare clients for independent living and increase their chances to sustain tenancies.
  • To monitor and evaluate a range of flexible and holistic treatment support packages to improve the quality of services delivered to clients
    in collaboration with a range of agencies
  • To develop and maximise service users’ personal strengths, social networks and recovery capital (social, physical, human and cultural) throughout their recovery.
  • To ensure that service users maintain regular health checks, including BBV and tuberculosis screening.
  • To fully inform service users about their recovery/treatment options, involve them in decisions and consent, and encourage them to take opportunities to achieve a sustained recovery.
  • To support ‘visible recovery’ within the service, promoting exit from treatment to service users at all stages of the recovery pathway.
  • To work collaboratively and proactively with other members of staff, volunteers and mentors to ensure that services are fully coordinated and are working collectively towards the achievement of recovery goals and positive outcomes for each individual client.

Organisational Responsibilities

  • To perform as part of the DHI team, attending and participating
    in regular team meetings, attend appropriate training courses.
  • To participate in regular support and supervision with the Team Leader.
  • To work with the Team Leader in the periodic review of the service policies and procedures.

Monitoring and Administration

  • Maintain accurate records and produce written reports of work
    as required.
  • Recording of all client information and interactions of up to date information on all clients.
  • To carry out other duties necessary for the smooth running
    of the service.

PERSON SPECIFICATION

Essential:

Experience of working within the area of substance misuse.

Experience of using motivation interviewing and brief solution focussed techniques.

Experience of partnership working and the ability to develop good working relationships with partnership agencies.

Excellent organisational skills, including managing and prioritising
a caseload with the ability to monitor and evaluate work.

Excellent communication skills.

Flexible, innovative and self motivated with a flexible approach to working hours in alignment with the Working Time Regulation.

Good IT skills.

Ability to work on own initiative as well as part of a team.

Ability to carry out assessments and care planning, in a client centred and multi disciplinary approach.

Ability to contribute to safeguarding and promoting welfare of children and vulnerable adults.

Experience of providing 1:1’s and/or group work
