  4. The parties agree to comply with the terms and conditions of the following exhibits, which are by this reference made a part of the Agreement.
    Standard Agreement #S1150002 between the Department of Personnel Administration and SAP Referral Services, LLC, effective July 1, 2011, is hereby amended as follows:
    Effective July 1, 2012, the Department of personnel Administration became the Department of Human Resources. This Amendment changes the contracting agency’s name and all Agreement references to the contracting agency from Department of Personnel Administration (DPA) to Department of Human Resources (CalHR).
    This Amendment exercises the second and third optional years to extend the contract term for two years from July 1, 2013 through June 30, 2015.
    The CalHR contact person cited in Exhibit A, page 4, is changed from Sydney Perry to:
