1.11 Record Keeping

...

  1. For all consumer services
  2. consumer referral and intake information, such as
  3. consumer name and social security number; and
  4. referral documents, such as pertinent medical, psychological, social, educational, vocational, and other information.
  5. financial records including, at a minimum
  6. invoices that
  7. match consumer records;
  8. contain service delivery date(s), type(s), quantity(ies);
  9. contain appropriate purchase order number; and
  10. contain printed name of the provider or the provider's signature; and
  11. records of DARS DRS payments.

  1. For Room, Board, and Supervised Living
  2. a copy of DARS3495, Vocational Rehabilitation Services Individualized Plan for Employment (IPE) from the referring counselor;
  3. records of consumer performance and attendance in sponsored services; and
  4. DARS3453, Monthly Progress Report—Work Oriented/Residential Community Rehabilitation Programs Monthly Progress Report.

5.For Job Quest Training

oa copy of DARS3495, Vocational Rehabilitation Services Individualized Plan for Employment (IPE) from the referring counselor.

oJob Application training - a copy of consumer data sheet and one job application;

oInterviewing Skills training - signed narrative report including evaluation (critique) of mock interviews;

oJob Seeking Skills training - narrative report by trainer;

oResume Development - copy of resume, cover letter, and references.

  1. For Job Placement
  2. a copy of DARS3495, Individualized Plan for Employment (IPE) from the referring counselor.;

oafter first day of work—a statement that the consumer is employed in a job; and.

  • after 90 days— a copy of a paycheck or paycheck stub, if available, and a statement.
  • a copy of DARS3430, Job Placement Services—Referral;
  • a copy of DARS3431, Job Placement Services—Plan;
  • a copy of DARS3432A, Job Placement Services—Support Summary, Benchmark A, After First Day of Paid Employment
  • a copy of DARS3432B, Job Placement Services—Support Summary, Benchmark B, After 45 Days of Paid Employment; and
  • a copy of DARS3432C, Job Placement Services—Support Summary, Benchmark C, After 90 Days of Paid Employment.
  1. For Vocational Adjustment Training
  2. a copy of DARS3495,DARS3495, Vocational Rehabilitation Services Individualized Plan for Employment (IPE) from the referring counselor.; and
  3. a signed narrative report.
  4. For Job Coach
  5. a copy of DARS3495,DARS3495, Vocational Rehabilitation Services Individualized Plan for Employment (IPE) from the referring counselor.; and
  6. DARS3458, Job Coach Service(s) Time Log, or equivalent.
  7. For Post-Acute Brain Injury Program
  8. a copy of DARS3495,DARS3495, Vocational Rehabilitation Services Individualized Plan for Employment (IPE) or DARS3496, Individualized Written Rehabilitation Plan (IWRP), Comprehensive Rehabilitation Services,Individualized Written Rehabilitation Program (IWRP) from the referring counselor;.
  9. a copy of the Individualized Treatment Plan signed by the
  10. DARS DRS counselor,
  11. consumer, and
  12. CRP case manager;.
  13. copies of progress reports (at least monthly);.
