DEMENTIA ACTION PLAN

2014 - 2016

INTRODUCTION

The following themes and actions were identified through a Dementia Engagement Workshop held in October 2013. A feedback session will be organised in March 2014.

All identified actions require cross-agency working, with the nominated lead agency to assume a coordinating role.

THEME: CARERS
3. Develop and disseminate information for carers, including BME carers, to include: recognising symptoms; information about dementia services available and how to access them; different referral points; dealing with challenging behavior; accessing carers support. / Increase in the number of contacts to key agencies and online portal. Number of people provided with information and signposted. Development of a Dementia website. / Jan 2014 / Better Care Fund
Review current training, and develop a cross-agency comprehensive training programme for carers on recognising symptoms, accessing services; managing care; and looking after themselves. / Number of people attending training and reporting that they feel more able to support the person with dementia to live in the community.

Review and consolidate available telephone information, advice and support for carers of people with dementia. / Successful establishment of a consolidated telephone service. Number of carers using telephone support and reporting it to be helpful and easy to access.

THEME: CARE AND RESPITE
Needs assessment to identify gaps/lack of capacity
Develop the local market to meet the needs of people with dementia and their carers, to include:
Home care, day care, night care, dementia cafes, care homes / Increased availability of a greater range of dementia-specific services. / March 2015
THEME: EASE OF ACCESS TO INFORMATION, ADVICE AND GUIDANCE (SCREENING TOOL),
Publicise information and guidance about dementia and dementia related services and pathways.
Produce a directory of services / People with dementia and their carers are able to find services, as evidenced by surveys.
Increase in the number of people accessing services / Funding will be required for the directory
Include the dementia ‘screening’ questions in the health checks programme / Sept 2014
Develop a website with general information for people with dementia and their carers

THEME: EASIER ACCESS TO DIAGNOSIS AND TREATMENT
Develop dementia friendly surgeries, e.g. prioritising appointments for patients and carers, longer appointments, reduced waiting times, GP training, screening carried out in primary care / GP practices are able to evidence dementia friendly surgeries / Higher level of patient and carer satisfaction, as evidenced by surveys / March 2015

THEME: ADVANCE CARE PLANNING AND END OF LIFE CARE
Support people with dementia and their carers with advance care planning, including wills, power of attorney, finances
Support carers with end of life care, and enable them to access End of Life (EOL) services and emotional support / Number of carers using EOL services, support and reporting they were helpful.
Target output: / 80% of carers using EOL services feel support and reporting they were helpful.
THEME: CARE COORDINATION, PREVENTING DUPLICATION
Improve care coordination for people with dementia through the integrated care agenda.
Develop clearer links between acute, mental health services, community services and social care when supporting people with dementia. / Better support across the dementia pathway, and higher level of user and carer satisfaction, evidenced by surveys. / n/a / March 2014

THEME: ADVOCACY

Review and develop dementia-specific practical support, e.g. making appointments, filling forms, dealing with agencies, getting help with transport costs. / Number of people using practical support services and reporting they feel more able to support the person with dementia to live in the community

Develop forums for carers of people with dementia to support each other and inform service provision. / Qualitative Survey-Number of people accessing peer support and reporting they feel more supported in caring for the person with dementia, and in expressing their views to service providers.
