6. BRIEF RESUME OF INTENDED RESEARCH WORK.

6.1 NEED FOR THE STUDY

Tundikeri is a highly prevalent disease in pediatric age from 5 -10years of age.1

Tonsils, the lymphoid tissue play important role in maintaining the immunity in children, hence timely treatment is most essential.2

Tundikeripresents with features of Kathina Shotha (enlargement of tonsils) resembling KarpasaPhala, Toda (pain), Ragatva (congestion), Galoparodha (dysphagia), Asyavairasya (distaste in mouth).3 Similar features are seen in tonsillitis as sore throat, difficulty in deglutition , unpleasant taste, bad smell.4As there is similarity in its presentation Tundikeri can be compared to tonsillitis.

The recurrent attack of Tonsillitis makes the disease chronic and vulnerable for infectious diseases.Several health hazards likelaryngeal edema, acute otitis media, quinsy, rheumatic fever, rheumatic heart diseases are often seen as systemic complications.5

Use of Saireyaka (Peeta)(Barleria prionitis) Moola Churna with Nimbuka(Citrus limon)Svarasa is a famous folklore medicine for Tundikeri in UttaraKannada and Dakshina Kannada regions of Karnataka state.

Saireyaka (Peeta)is Tridoshaghna especially Vata-Rakta-Kaphagna6 in action which exactly correlates with Doshic configuration of Tundikeri7.Similarly NimbukaSvarasa hasVata-Kaphahara property with special indications in Kaphotklesha, Asyavairasya and Amajanya Vyadhi8.

Considering the above points, the present study is planned to evaluate the effect of Saireyaka Moola Churna with Nimbuka Svarasa (as oral medication) in management of Tundikeri.

6.2 REVIEW OF THE LITERATURE

Tundikeri isone among the Urdhvajatrugata Roga; mentioned in Talugataroga by Acharya Sushruta9 and Kanthagataroga by Acharya Vagbhata10.

Lakshanas of Tundikeri are Kathina Shotha (enlargement of Tonsils) resembling Karpasa Phala, Toda(pain), Ragatva (congestion), Galoparodha (dysphagia), Asyavairasya (distaste in mouth) etc.11

Specific Nidana of Tundikeri is not told in Samhitas but Samanya Nidana of Kanthagata Roga like Excessive consumption of Anupa Mamsa, Ksheera, Dadhi, Masha are considered as causative factors for Tundikeri. Peoplepracticing Avakshayya (sleeping with downward face), avoidingDanta Dhavan, Dhumapana and Gandusha are more prone to Tundikeri.12

Classics have mentioned Samanya Samprapti of Kanthagata Roga, which can be taken for understanding the pathogenesis of Tundikeri. The Dosha causing Tundikeri are Kapha Pradhana with Rakta Anubandha13, 14.

Tonsillitisis inflammation (swelling) of thetonsils presenting with features of difficulty in swallowing, earpain,fever, sorethroat, tenderness of the jaw and throat.15

Children are usually treated with a course of Antibiotics and such 4-5 attacks are justifiable for Tonsillectomy.16

Saireyakais of 4 varieties; out of which Peeta Saireyaka has Tikta-Madhura Kinchit Amla Rasa, Usna Virya, Laghu Guna, Katu Vipaka.17 This drug brings down the symptoms likeShotha, Vidaha, Kandu etc.18, 19

As it is mentioned in Aragvadhadhi gana in classics; it has Jwarahara, Vishagna and Vrunashodhana properties.20, 21

Nimbuka svarasa which is used to make Kalka has Amla-Katu Rasa, Laghu-Tikshna Guna,Usna Virya, Amla Vipaka.22 It has Vaktrashodhana, Rochana, Shoolagna properties. It cures Aruchi and cures various Vata Rogas.23

7.MATERIALS AND METHODS

7.1 SOURCE OF DATA

Patients will be selected from the Outpatient Department and Inpatient Department of Kaumarabhritya; S.D.M. College of Ayurveda and Hospital, Hassan

7.2 METHOD OF COLLECTION OF DATA.

DIAGNOSTIC CRITERIA

Diagnosis will be made on the basis of Symptoms of Tundikeri like Ragata (congestion), Galoparodha (dysphagia), Kathina Shotha (enlargement of tonsils), Toda (pain), Asyavairasya (distaste in mouth).

INCLUSION CRITERIA

1. Children of age group of 6-10 years.

2. Children of both sex.

3. Children with history of Tundikeri for more than 3 months.

EXCLUSION CRITERIA

  1. Patients with Peritonsillar Abscess, Tonsillar Cyst,Tonsillolith will be excluded.
  2. Patient associated with any other systemic disorder.

RESEARCH DESIGN

Sample size:

30 patients of Tundikeri will be taken for clinical trial on both OPD and IPD basis.

Procedure:

Saireyaka (Barleria prionitis) Moola Churna 4grams mixed with 5 ml of Nimbuka Svarasa (lemon juice)will be given orally,before foodtwice a day in the morning and evening.

Duration of Treatment:7 days.

Follow up:After completion of treatment,the patients will be called for follow up after the gap of 10 daysfor one month.

ASSESSMENT CRITERIA

Suitable scores will be assigned to give some objectivity to the subjective symptoms to assess improvement in the signs and symptoms of Tonsillitis like Galoparodha (dysphagia), Asyavairasya (distaste in mouth), Kathina Shotha (enlargement of tonsils), Toda (pain). Objective parameters will includeregression in the size of tonsils using suitable parameters.
