Dossier Requirements for Mid 10-Year Review

These guidelines have been adopted to prescribe thecontent and format of Mid 10-Year Review Dossiers. They are intended to ensure that Mid 10-Year Review Dossier materials are transmitted in a consistent format to aid in efficient review and decision making. Please ensure that all requirements are met before submitting the dossier. Dossiers that do not meet these requirements will be returned for correction.

Candidate Materials:

Faculty Affairs will provide a link to the Mid 10-Year Review Dossier to faculty undergoing Mid 10-Year Review. The dossier will be pre-formatted with the required folders.

Candidate materials should be saved to the appropriate folder in the Mid 10-Year Review Dossier as described below. All Candidate materials must be uploaded to the dossier by October 1.

Dean & Peer Review Committee Materials:

Deans and Peer Review Committee Chairs should submit dossier materials to Faculty Affairs via by published deadlines. Faculty Affairs will upload those materials to the Mid 10-Year Review Dossier.

Description of Dossier Materials

  1. Self-Evaluation
  • Prepared by Candidate as part of Mid 10-Year Review Dossier
  • Required Form: Self-Evaluation Template for Mid 10-Year Review
  • Due Date: October 1
  • Save in “Self-Evaluation” folder in the Mid 10-Year Review Dossier

The self-evaluation serves as the foundation of the dossier and Mid 10-Year Review. Using theSelf-Evaluation Template for Mid 10-Year Review, the faculty member will first provide his/her Philosophy of Teaching Statement. He/she will next provide a self-assessment of his/her activities and performance for the contract period under review. This self-assessment should include an overview of teaching, creative activity/research, and service.

  1. Curriculum Vitae (CV)
  • Prepared by Candidate as part of Mid 10-Year Review Dossier
  • Due Date: October 1
  • Save in “Curriculum Vitae” folder in the Mid 10-Year Review Dossier

The most recent version of the faculty member’s CV. It should be dated and paginated. It should include a list of courses taught, a list of administrative assignments and professional creative activity/research.

  1. Student Course Evaluations
  • Prepared by Candidate as part of Mid 10-Year Review Dossier
  • Due Date: October 1
  • Save in “Student Course Evaluations” folder in the Mid 10-Year Review Dossier
  • Each set of course evaluations should be saved as a separate document
  • Create academic year folders for each academic year. Create semester folders within each academic year folder.
  • Student Course Evaluations

These are the faculty member’s Student Course Evaluations for the period under review. The faculty member should check with the Dean’s Office if he/she does not have copies of his/her student course evaluations for the period under review.

  1. Syllabi
  • Prepared by Candidate as part of Mid 10-Year Review Dossier
  • Due Date: October 1
  • Save in “Syllabi” folder in the Mid 10-Year Review Dossier
  • Each syllabus should be saved as a separate document
  • Create academic year folders for each academic year. Create semester folders within each academic year folder.
  • Syllabi

  • Fall 2014
  • Spring 2015

These are the faculty member’s syllabi for courses taught during the period under review.

  1. Annual Evaluations
  • Prepared by Candidate as part of Mid 10-Year Review Dossier
  • Required Form: Annual Evaluation/Update of Professional Activities Form, Parts I & II
  • Due Date: October 1
  • Save in “Annual Evaluations” folder in the Mid 10-Year Review Dossier
  • Part I and Part II of each evaluation should be saved as one document
  • Each evaluation should be saved as a separate document. Do not combine multiple years into one document

The faculty member’s Annual Evaluation/Update of Professional Activities Form, Parts I & II for each year of the period under review. The faculty member should check with the Dean’s Office if he/she is missing either Part I or Part II of the Annual Evaluation.

  1. Supplemental Support
  • Prepared by Candidate as part of Mid 10-Year Review Dossier
  • Due Date: Candidate may save in dossier until Oct 1. After that date, Candidate may submit any additional materials to Faculty Affairs to be included in the dossierat any time.
  • Save in “Supplemental Support” folder in the Mid 10-Year Review Dossier

This includes any additional information in support of the review. This item can be updated at any time. However, materials submitted after a stated dossier submission deadline does not affect subsequent deadlines and does not cause the Peer Committee, the Dean or the Provost to be obligated to re-evaluate a faculty member.

  1. Peer Review Committee Evaluation
  • Prepared by Peer Review Committee
  • Required Form: Peer Review Committee Evaluation for Mid 10-Year Review
  • Due Date: Peer Review Committee Chair submits to Faculty Affairs via by December 15 for inclusion in the Mid 10-Year Review. Peer Review Committee Chair sends a cc to the Dean.

The Peer Review Committee Evaluation for Mid 10-Year Review addresses:

  • the three areas of evaluative criteria: creative activity/research, service, and teaching. The Peer Committee should indicate their assessment of the faculty member in each area according to the guidelines outlined in the UNCSA faculty manual
  • Peer Review Committee members’ classroom observations (e.g. classroom, studio, lab)
  • student evaluations for each year under review

  1. Dean’s Evaluation
  • Prepared by the Dean
  • Required Form: Dean’s Evaluation for Mid 10-Year Review
  • Due Date: The Office of the Dean submits to Faculty Affairs via by February 15.

The Dean submits the Dean’s Evaluation for Mid 10-Year Review to the Provost. This document addresses:

  • the three areas of evaluative criteria: creative activity/research, service, and teaching. The dean should indicate his/her assessment of the faculty member in each area according to the guidelines outlined in the UNCSA faculty manual
  • the dean’s instructional observations (e.g. classroom, studio, lab)
  • student evaluations for each year under review
  • recommendations and expectations for future growth and development

  1. Acknowledgment Statement
  • Prepared by the Dean for Candidate signature.
  • Required Form: Acknowledgement Statement for Mid 10-Year Review
  • Due Date: The Office of the Dean submits to Faculty Affairs via by February 15.

The Acknowledgement Statement is the Candidate’s acknowledgement that he/she has reviewed the completed Mid 10-Year Review Dossier and discussed the evaluation with the Dean. The Office of the Dean must complete theAcknowledgement Statement for Mid 10-Year Review and have it available for the faculty member’s signature when the Dean meets with the Candidate to discuss the evaluation.
