DONATION BOX REQUEST FORM
A registered student organization who is in good standing may request to host a donation box in the Student Center based on the following policies:
· A student organization may host a donation box up to two times a semester for a maximum of one week (beginning Sunday, ending Saturday) at a time.
· All reservation requests for donation boxes should be inputted through the Book It @ TCNJ system. Once the student organization receives the reservation confirmation they are responsible for filling out this form and handing into the Brower Student Center Information Desk no less than 2 business days before their reservation begins. Any organization that does not hand in this form within 2 business days will not be allowed to display their donation box and will be required to re-schedule their reservation for a later date.
· The student organization must collect donations at least once per day during the duration of their reservation and maintain the appearance of their donation box. All donations boxes must be appropriate and professional in appearance and be decorated to not look like a cardboard box.
· All donation boxes must have clear signage on it to reflect the name of the student organization, the cause they are collecting for and the dates of the donation drive.
· The organization must remove their donation box at the end of their reservation. Student organizations who do not pick up their donation box at the end of their reservation jeopardize future donation box privileges.
· All donation boxes must be placed in front of the Information Desk in the Student Center atrium.
· There will be one donation box per organization per reservation.
· Only two student organizations may host a donation box display during the same week period.
· Any organization that sets up a donation box without prior approval will have their donation box discarded.
· Failure to abide by any of the above policies will result in suspension of donation box privileges for the remainder of the academic year and/or the following academic year if violation occurs at the end of a Spring semester.
· Please note that the Student Center is not responsible for item contents within the donation box or the donation box itself.
