Dog Class Agreement
I understand that attendance of a dog training class is not with out risk to myself, members of my family or guests who may attend, or to my dog(s), because some of the dogs to which I will be exposed may be difficult to control, and may be the cause of injury, even when handled with the greatest of greatest degree and amount of care.
By attending dog training classes and the execution of this agreement, I expressly agree that, K-9 Academy,LLC, Rolla Animal Hospital and it’s members, it’s instructors, it’s staff, and it’s agents shall not be liable for any damage arising from personal injury or other damage or injury sustained by me, my guests, or my dog, in, on, or about the premises of Rolla Animal Hospitalor any other location where said training classes may be held, or as a result of attending, observing, or participation in training classes. I assume full responsibility for any such injury or damage that may occur to me, my guests, or my dog, in, on, or about the facilities and further agree K-9 Academy, LLC, Rolla Animal Hospitalshall not be liable for any loss of theft of personal property. I also specifically agree that K-9 Academy, LLC or Rolla Animal Hospital, shall not be responsible for any such injuries, damage, loss, or theft even in the event of negligence byK-9 Academy, LLC, Rolla Animal Hospital, whether such negligence is present at the signing of this agreement or takes place in the future. This waiver does not, however, apply to gross negligence or intentional torts byK-9 Academy, LLC, Rolla Animal Hospital. I further agree to indemnify and hold harmless K-9 Academy, LLC, Rolla Animal Hospital, injury, loss, or theft, including, but not limited to a reasonable attorney’s fee, caused by myself, my guest, or my dog while on the premises of Rolla Animal Hospital, or at any other location which such classes may be held. I further agree that this agreement is binding for this and any subsequent class, which I may attend.
I give K-9 Academy, LLC permission to post pictures of my myself, my dog, family and friends to the internet and social media (facebook, twitter, K-9 Academy website, etc).

When attending class please make sure your dog has a collar on. Also, please bring treats and lots of enthusiasm to each class. Also, since we will be rewarding good behavior with treats please try not to feed your dog approximately 3 hours before each class.
Refund Policy: If attend one class you will receive %90 refund. If you attend 2nd class you will receive %75 refund. After 2 full classes have been missed no refund will be made. (must return all equipment, otherwise equipment fee will be taken out of refund. Equipment must be deemed reusable by K-9 Academy, LLC to get full equipment refund).

K-9 Academy Accepts Credit Cards (Mastercard, Visa and Discover), cash and checks for payment.

Return Check policy: Returned check will incur a $25 fee.
