Dog Adoption Contract

A. IAR agrees to:

1. Transfer the dog to the Adopter.

2. Provide a record of vet care that was provided while the dog was in foster care.

3. Provide heartworm preventative and flea/tick preventative while in foster care.

4. Provide routine intestinal deworming while in foster care.

5. Providedistemper/parvo and bordetella vaccines while in foster care.

6. Provide a rabies vaccine for all dogsthat are 16 weeks old or older prior to time of adoption.

7. Provide spay or neuter for the dog prior to adoption, (or arrange by16 weeks if adopted prior to 16 weeks)

8. Provide a microchip.

9. Assist with advice and/ora referral to appropriate training for the adopted dog as needed.

10. In the case that the adopter cannot or will not continue to provide a home for the dog, IAR will consider the dog for re-entry into the IAR foster program. In such cases, the dog will be given a behavioral assessment, and a thorough history will be required. In order for a dog to be a candidate for re-entry, the dog must pass our behavioral assessment and have no history of aggression.

B. IAR prohibits, and the Adopter agrees not to do the following:

1. Confinement of dog by chain, rope, trolley or allowing dogs to run at large.

2. Debarking of dogs.

3. Cosmetic surgery such as ear cropping and tail docking.

C. The Adopter agrees to:

1. Adopt the dog as a personal pet or companion, not as a gift, not as a hunting dog, not as a guard dog, and not to beused for research purposes.

2. Make every effort to give the dog a life-long home, including being proactive with behavioral training, socialization, and planning in advance for the dog’s needs if you move or make other changes in your living situation.

3. Provide all veterinary care and treatment needed by the dog, including yearly medical checkups, yearly fecal checks, anynecessary vaccinations indicated by the veterinarian, yearly heartworm tests, monthly heartworm preventative, monthlyflea/tick preventative, and immediate treatment of illnesses or injuries.

4. Obtain permission from landlord (if renting) and pay any required pet deposits or fees prior to adoption.

5. Provide adequate fresh food and water, clean and dry shelter, and daily exercise.

6. Provide indoor privileges and frequent human interaction.

7. Provide leashed or fenced access to outdoors several times per day (or more!) for the dog’s urination and defecation needs.

8. Provide a safe collar with rabies and identification tags.

9. Attest that all other pets/animals in household are spayed/neutered and current on vaccinations.

10. Fully and completely releaseIAR, its agents and volunteers from any claim of liability for illness or injury the Adopter or the Adopter’s animals may develop from the IAR animal.

11. Obey all applicable federal and state laws governing the control and custody of animals.

12. Pay the nonrefundable adoption fee.

13. Allow IAR to visit my premises to insure that the terms of this agreement are met.

14. Contact IAR if you are unable or unwilling to continue to provide a home for the adopted dog.

15. Pay the costs, including attorney and court fees, if IAR must take action against the Adopter or his/her representatives to enforceany terms of this contract.

ENFORCEMENT OF AGREEMENT:

I give my permission for an agent of IAR to remove the animal from my premises if any terms of this agreement are violated by the Adopter,and such entry shall not constitute a trespass upon the premises occupied. ______(initials).

RELEASE AND WAIVER

I, the undersigned, understand that the adoption of a pet animal carries with it certain risks, including but not limited to, thetransmission of such diseases as rabies, among others, and animal bites, scratches or attacks to humans or other animals, whichmay result in serious bodily injury and even death. I hereby assume all risks associated with adopting a pet animal fromIndependent Animal Rescue, including, but not limited to, transmission of such diseases as rabies, among others, animal bites,scratches and attacks to humans or other animals, all such risks being known and appreciated by me. Having read this releaseand knowing these facts, and in consideration of IAR allowing me to adopt a pet animal, I, for myself, my heirs, assigns andanyone else entitled to act on my behalf, do waive, release and hold harmless IAR and its affiliates, and their respective directors,officers, and employees, or their successors, from all demands, causes of action, judgments, expenses or losses and claims ofliabilities of any kind, including injury or death (the “Claims”), arising out of my adopting a pet animal through IAR, even thoughsuch Claims may arise out of negligence or carelessness on the part of such entities/persons.

The Adopter verifies that he/she is 18 years of age or older and fully understands the contractual obligations set out above.
