DOCUMENTS REQUIRED TO APPLY FOR A NATIONAL VISA
(D visa →91 to 365 DAYS)
PURPOSE OF TRAVEL: FAMILY REASONS
Family members of foreign citizens with residence permit or work visa for subordinate employment, self-employment, study or religious reasons valid for no less than one year, as well as residence permit for Asylum and Family Reasons.
Please note that to issue a subordinate work visa this Consulate must have received the Nulla Osta from the Sportello Unico Immigrazione (Italian Immigration Authorities)
Document check-list
1. National Visa application form: all fields in theform must be filled, the names appearing on the passport must match the ones on the form. The applicant must date and sign the form before the Visa Officer.
2. Recent passport size picture (3.5 x 4.0 cm): white background, full face and front view. Scanned or photocopied photos will NOT be accepted;
3. Passport and a photocopy of its main pages, as well as of any other pages with visa stickers and Airport Authorities’ stamps;
NOTE: your passport must be valid at least for 90 days beyond the expiry date of the visa, must have at least 2 blank pages and must have been issued not more than 10 years before.
4. Your Permanent Resident Card (original plus photocopy of both sides) if applicable
5. Your Driver’s License (original and Photocopy) or any other proof of residency as proof of residence in the jurisdiction of this Consulate (Quebec, New Foundland and Labrador, Nova Scotia, New Brunswick, PEI, Nunavut).
6.Copy of the Nulla Osta issued by the Sportello Unico Immigrazione (Italian Immigration Authorities). This document is given to the applicantby the SUI and must be used within six months of its issuance;
7. Marriage/birth certificate attesting the family relationship;
8..Confirmed flight reservation showing complete itinerary;
9.Visa fee (non refundable) in the form of cash or Money Order, payable in Canadian Dollars to the Consulate General of Italy in Montreal, with applicant’s name and address as the remitter. Personal Checks will not be accepted;
10.Return envelope:If you prefer to have your Passport with the visa mailed to you, please provide a Pre-paid self addressed Express Post envelope (available at Canada Post offices).
PLEASE NOTE:
The Visa Office has full authority to evaluate and request other documents in addition to those submitted by the applicant. The applicant is hereby informed that submitting the documents does not guarantee issuance of the visa.
PURPOSE OF TRAVEL: FAMILY REASONS
Family members of EU or European Economic Area citizens
European Parliament and Council Directive 2004/38/EC of 29April2004 on the right of citizens of the Union and their family members to move and reside freely within the territory of the Member States.
- Family Members: the spouse; the registered partner, if the legislation of the host Member State treats registered partnerships as equivalent to marriage; the direct descendants who are under the age of 21or are dependants and those of the spouse or partner as defined above; the dependent direct relatives in the ascending line and those of the spouse or partner.
Starting August 8th 2011, family members of EU citizens no longer require a Family Visa to move and reside in Italy for more than 3 months.
- NON EU Nationals who do not require a visa to enter Italy must register with the competent authorities (Questura or Comune) to obtain the Residence Permit for EU family members.
- Nationals who require a visa to enter Italy must apply for a Schengen Visa for tourism. The documents required for the Visa are:
