Team Planning Tool for Student-Focused Planning and Student Development Practices
Team: Meridian
Team members: Mike Shuttie, Gina Ferman, Letha Bauter, Cindy Branscum, Jana Collins, Anne Belanus, Judy Stanton, Kim Dowdy, Tawanna Partridge, Nancy Moss, and Melisa D. Kifer
Focus #1: Student focused planning to create a seamless transition between secondary and post secondary options
Goal: Develop student-directed transition focused IEP’s
Specific Goal-Related Activities:
Survey to be distributed to middle and high school special education teachers regarding use of age appropriate, individualized transitional assessment material (including career, academic and self determination) and use of the information obtained as a tool in developing IEP’s
(Note: The responsible parties of Letha Bauter, Kim Dowdy, and Anne Belanus will send out the initial survey by August 2, 2006 with the goal of having all surveys returned by September 15, 2006.)

Expected Outcomes
- Increased student, teacher, family, service provider knowledge of students’ transition-related skills and / or behaviors
- Awareness of other tools available

Goal: Develop student-directed transition focused IEP’s
Specific Goal-Related Activities:
Regional meeting hosted by Meridian to provide venue to discuss results from the survey, share assessments available including career, academic and self determination
Anne Belanus to provide training on the topic of using assessment as a tool for developing readiness and skills for post secondary options
Inviting special education teachers, service providers, and key stakeholders within the community to the regional meeting.
(Note: The responsible parties are all Meridian team members and will be held around September 15, 2006.)
