Enrollment Agreement
We, the undersigned parent(s)/guardian(s) of the above-named child (hereinafter referred to as “Member”), understand and agree to the following terms and conditions of enrollment of our child at My First School:
I.Member’s Responsibilities
Member acknowledges that My First School is a cooperative school designed to offer Members the opportunity to be actively involved in their children’s introduction to school while at the same time affording a quality classroom experience with well qualified teachers. Member further acknowledges that in order for My First School to provide this service it requires the participation of all members in the operation of the school. Therefore Member agrees to participate in School operations as follows:
✓ Member agrees to act as an assistant to the teacher in the class that Member’s child is enrolled in on an alternating basis with other Members, unless the opt out option has been taken. In this regard it is understood that: how often Member will be required to assist will depend on class size; Member will assist for the entire class period; Member will not bring sibling(s) when assisting; in the event Member cannot attend on a day that he/she is scheduled to assist, Member will be responsible for contacting another Member (Class Coordinator first) to act as a substitute.
✓Member understands and agrees that in order for My First School to continue to provide a quality experience to Members and their children it requires Members to assist in the administration of the School and/or to participate in various fundraising activities that take place throughout the school year. These administration and fundraising activities are organized and implemented by Members divided into separate committees that regularly meet, plan and perform the services necessary in order to accomplish each Committee objective. Member agrees to serve on at least one Committee (unless Member’s child is only in the Bumblebee or TT classes) to be assigned at the beginning of the school year. Alternatively, Member may opt out of this requirement by paying to the School a non-refundable $250 non-participation fee payable by October 1st of the school year. At the discretion of the Executive Board of the School, a non-participation fee may also be assessed at any time during the year for failure to meet Committee obligations.
✓In addition, Member acknowledges that he/she has read and accepts all the policies and procedures in the current My First School parent handbook.
II. Enrollment
✓In order to reserve a place for your child, this agreement must be signed and returned to My First School together with a non-refundable enrollment fee of $125 payable in cash, check or money order. Upon acceptance, a copy of this agreement will be placed in your school folder. Copies can be given upon request. An annual physical exam report for the child verifying all NYS immunizations must be on file by August 1, 2017.
✓ Acceptance of enrollment is subject to the following priorities: (1) a child who has previously been enrolled will have first priority; (jJan. 5th) (2) a child who is a sibling of a child who has been previously enrolled will have next priority, and (3) the child of an Alumni Member /WMC member will have final priority.(Jan. 13h) Priority enrollment will occur by submission of enrollment agreements with non-refundable enrollment fees during the period January 5, 2017 through January 12th 2017. As of January 13, 2017, enrollment will be open to the public on a first come first serve basis.
✓ Notwithstanding the above, Member understands and agrees that the Executive Board, in its sole discretion, reserves the right to cancel, combine or alter a class based on the needs of the group, demands on the teacher’s time, and the needs of each child.
III. Tuition and fees
Annual tuition for the 2017-2018 school year is as follows:
✓ A $125 non-refundable enrollment fee
✓My child ______is enrolling in the ______class with anannual fee of $______. We ____will be/____will not be participating as a helping parent. Tuition payments will be made in 10 monthly installments ($______)starting on April 10, 2017 through January 10, 2018. A 5% discount will be given to parents who pay tuition in full by August 1, 2017. All payments must be made by cash, check or money order.
✓ A $25 late fee may be imposed on payments not received by the 15th of the above-mentioned months.
✓ A $30 fee will be imposed for any returned checks for insufficient funds.
✓Scholarship forms (downloaded online or picked up in director’s office) must be returned by 10/1/17. Recipients notified by 11/1/17.
IV. Voluntary Withdrawal
In the event of voluntary withdrawal from the School the following will apply:
✓The $125 non-refundable enrollment fee will be forfeited unless the following circumstances apply:
✓A serious or prolonged illness of either the Child or the Child’s primary caretaker (other than a nanny, au pair, baby-sitter, or the like) as documented by a physician. In this circumstance, Member may request a refund in writing to the Executive Board.
✓A child withdrawn prior to the start of school in order to enter a center-based special education program. In this circumstance, Member should cause the center-based special education program to send My First School a verifying letter.
✓ Parents must provide written notice to the Executive Board of withdrawal for any reason. In these cases, tuition will not be refunded unless another child is found to replace the Member child’s spot, or upon a unanimous Executive Board decision. In this case, the Executive Board will determine what portion, if any, will be refunded. Whenever a child attends class for any part of a given month, there will be no refund consideration for that month.
V.Absence and school closings
There will be no reduction or refund of tuition for absences, school closings or holidays as tuition is an annual fee. In this regard, Member understands that the school calendar follows the calendar followed by the Floral Park/Bellerose Public School but classes may be cancelled and/or rescheduled on an emergency basis at the discretion of the Executive Board and the School Director.
VI.Dismissal of child from enrollment
My First School reserves the right to dismiss a child from the School if it is determined by the Professional Staff or Executive Board to be in the best interests of the child’s class or if Member fails to make any tuition payment within thirty (30) days after its stated due date. Upon such a dismissal, My First School will calculate and refund that portion of tuition that has been paid for the remainder of the school year.
