CHILD ABUSE PREVENTION (CAP) MONTH
GRANT APPLICATION AND PROGRAM AGREEMENT
Grants may range from $500 to $1500 based on each community’s plan, proposed budget, and the total number of statewide grantees.
Instructions for submitting a request for a 2016 CAP Month Grant
Deadline for Submission: December 18, 2015
1) Project Narrative: In one to two pages provide the following information in narrative form.
Contact Information. Name, phone number, e-mail address, and title of individual whothat will be heading the project.
Community Needs. Current needs in local communities your grant will address and the reasons your team feels these issues are important.
Goals and Outcomes. What you intend to achieve in your efforts.
Proposed activities and projects. Identify what you will do to meet your goals. Specifically, wWho is your intended audience, and how many people will be served?. Please describe how your team will ensure that activities are provided to the population in your area and how you will expand activities beyond the month of April.
Materials Requested. Will you need yards signs? If so, how many?
Brochure Themes. Please indicate the topic area(s) that correspond with your activities:

Parenting ___ Fatherhood___ Community Involvement___ Family Conflict___ Child Discipline___

Evaluation. Tell us how you will know that your efforts have been successful.
Sponsoring Agency. Include the contact person, agency, telephone number, email address, and mailing address of the individual and agency responsible for coordinating activities, requesting reimbursement, and completing the final report.
Fiscal Agent. If different fromthan the sponsoring agency, include the agency, individual contact person, telephone number, email address, and mailing address of the individual/agency responsible for managing the funds.
Community Partners. Please note all participating partners.
Service Delivery Area. List towns, counties, or regions your funding request will serve.
2) Prepare the formal grant application by listing the following.: Please use the form provided.
Budget: Describe in detail how grant funds will be expended. Food, snacks, drinks,, or t-shirts are not an allowable cost.
CAP Coordinator: If you intend to use CAP grant funds to support project coordinator time, please identify that person along with their contact information.

Prevent Child Abuse North Dakota will provide:
· Logistical support as needed;
· Web-based CAP Month activity calendar and partner support;
· Yard sSigns (for NEW gGrantees);
· Template for various media submissions;
· Educational brochures/booklets;
· Materials for display/distribution (wristbands, pencils, crayon color wheels, etc.); and
· Web- based templates for information distribution.
The following items are requirements of CAP funding* use:
· Inter-disciplinary community/regional planning group.
· Adherence to PCAND requirements for templates, messaging, and referencing PCAND and DHS on all printed materials.
· Use of media by way of letters to the editor, public service announcements, signage, and interviews relating to activities and parent support programming availability.
· Agreement to submit a Final Report on activities and number of people reached along with reimbursement requests by May 20, 2016. Please submit all reimbursement requests together at the same time.
**This funding is specifically intended for use to address the prevention of child abuse and neglect (i.e. person responsible for the child’s welfare). Issues such as bullying, dating violence, human sex trafficking, etc. can be subtopics of CAP Month efforts, but should not be the main focus of CAP Month gGrant-funded aActivities.
Suggested Menu of Activities:
Based on your needs assessment, sSet up activities relevant to the needs of the community, based on the needs assessment s. Such as:
o Partner with local faith communities churches to create an Sunday event/sermon/flyer honoring families and those who work on behalf of children and families.
o Set up an informational booth or table at a well-traveled location or in conjunction with another activity.
o Partner with hospitals/clinics to provide education to new and expectant parents.
o Set up classes relevant to the needs of the community.
o Partner with businesses/organizations and provide an educational fair.
o Host a “Prevention Walk.”
o Partner with local restaurants to bring messages of positive parenting.
o Partner with schools for a drawing/coloring contest.
o Use your imagination for other prevention activities that are suitable for your community.

Final Report/Evaluations
A final report of grant activities, evaluations of events (including approximate numbers of participants/individuals reached), and requests for reimbursement must be submitted together to PCAND by May 20, 2016. Any photos (preferably digital) of events and pinwheel gardens or copies of news articles or materials developed are greatly appreciated.

Exciting Opportunity for CAP Grantee Applicants: Caring Community Award 2016
During CAP Month Earlier this year, 2015, PCAND hostedeld the 2nd Annual Caring Community Award Event. A Caring Community is a neighborhood, school, organization, or business that nurtures the safe and loving development of children and has committed the time and effort to educate itself on how to prevent, recognize, and react responsibly to child abuse.
Awardees will be publically recognized at the Governor’s Proclamation of April as Child Abuse Prevention Month. Additionally, awardees will be spotlighted on PCAND’s website and social media, Facebook page as well as featured in a press release.
