OSU Scientific Diving Program Requirements
The following is an outline of the basic requirements for status as an OSU Scientific Diver. Additional individual requirements may be added at the discretion of the Diving Safety Officer (DSO) and/or Diving Control Board (DCB). OSU is an organizational member of the American Academy of Underwater Sciences (AAUS), and OSU scientific diving requirements meet or exceed current minimum scientific diving standards set by AAUS. Applicants should initially contact the DSO for review of diving application and medical history.
· Completed OSU scientific diving application (includes copy of personal dive log)
· Current OSU/AAUS Diving Medical History form (diver supplied)
· Current OSU/AAUS Medical Evaluation of Fitness for Scuba Diving (medical exam with tests initialed by the physician, signed by the physician, with physician contact info provided)
· Ability to complete OSU swimming evaluation (Sec. 4.10 of OSU Diving Safety Manual)
· Proof of diving certification from a recognized training agency (Advanced and/or Rescue certifications strongly preferred)
· Proof of completion of a scientific diving course meeting minimum AAUS practical skills and academic knowledge standards (experience in lieu of training determined individually by DSO and/or DCB)
· Current certifications in CPR, First Aid, and AED (must include a practical component)
· Current certification in Emergency Oxygen administration
· Proof of diving accident insurance coverage (DAN accident insurance or statement from equivalent provider)
· Proof of diving equipment service within the last 12 months
· Final OSU Scientific Diver approval granted by DCB
To Maintain Active Status:
· Satisfactory maintenance of above requirements and adherence to all OSU scientific diving policies
· Diving activity: minimum 12 OSU dives within the last 12 months
· Regular submission of dive logs to OSU DSO (annual reporting is minimum)
