How do I request a refund of premiums paid for September 2017 Evergreen Health coverage?

/ To request a refund, email a completed copy of the Premium Refund Request Form along with validation that your group enrolled with another carrier to or send the completed form to:

What documents can I submit to validate that my group enrolled with another carrier?

/ Acceptable documents for validating enrollment with a new carrier include: welcome letter, or other document from the new carrier showing the group’s date of enrollment.

How will I receive my premium refund?

/ All refunds will be paid by check regardless of the method of payment used to pay the premium.

When should I expect to receive my premium refund?

/ The Receiver expects to process requests for refunds within 30 days of receiving the necessary documentation.

Is the refund request process different for groups that paid their premium to a broker or third party administrator (“TPA”)?

/ No, all September 2017 premium refund requests should follow the process outlined above.

If premiums were paid through a TPA, will the refund go back to the TPA or to the group?

/ Premiums for groups that paid through a broker or TPA will be refunded to that broker or TPA. Groups that did not pay through a TPA will be refunded directly by Evergreen.

Can a group that paid premiums for August 2017 and enrolled with a new carrier in July 2017 request a refund for the August 2017 premium?

/ The Receiver has authority from the court to issue refunds for premiums paid for September 2017 and later periods. Proof of claim forms will be provided at a later date to request premium refunds for periods prior to September 2017. For more information on this process go to www.evergreenmd.org.
