Objectives

  1. Compare and contrast the roles of the staff nurse and Unlicensed Assistive Personnel (UAP) in the long-term care setting.
  2. Identify a current Organizational Chart in the clinical setting.
  3. Identify an example of the Chain of Authority in use at the clinical site.
  4. Identify an active Interdisciplinary Plan of Care in the clinical setting.
  5. Demonstrate ability to devise an individualized Concept Care Diagram that is based on an active facility Plan of Care.
  6. List effective Teamwork and Collaboration strategies in use at the clinical site.
  7. Recommend techniques to improve Interdisciplinary Team group-work.
  8. Observe techniques with negative impact on Interdisciplinary Team group-work.

Clinical Activities

  1. Identify how the staff nurse’s role on the Healthcare Team is different from the Unlicensed Assistive Personnel. Use the Venn diagram to compare and contrast the roles.

  1. List effective Teamwork and Collaboration strategies utilized by the Healthcare Team on the clinical unit.
  1. Compare information obtained for three different residents:

Resident #1 / Resident #2 / Resident #3
List the members of the healthcare team involved in 3 different patient’s care.
Identify the overall goal of the healthcare team for the 3 patients.
  1. Interview a member of the Interdisciplinary Team (other than nursing) and the Case Manager about the facility Plan of Care for your assigned resident. Record key points:

  1. Identify which concepts are addressed in the facility Plan of Care for your assigned resident.

  1. Find the Organizational Chart or create one that outlines Chain of Authority at the clinical facility. Make a rough diagram on a separate sheet of paper.
  1. List three examples related to Chain of Authority observed at the clinical site.

Interdisciplinary Team Meeting

(Instructor may omit this section if student is unable to attend a meeting.)

Attend an Interdisciplinary Team Meeting (often combined with or in place of a “Care Plan Meeting”) and observe the interaction of the members.

  1. Make a list of what you observe that facilitates positive teamwork during the meeting.
  1. Make a list of any techniques we have discussed in lecture that might be a positive addition to what you observed.
  1. Identify any communication techniques that had a negative impact on the group-work.
